Sabe-se que estes colirios aumentam a pressao intra-ocular tanto dos olhos glaucomatosos como dos nao glaucomatosos por meio de mecanismos discutiveis. O metodo de biomicroscopia ultra-sonica permitiu mostrar as relacoes anatomicas das estruturas do segmento anterior do olho humano in vivo qualitativa e quantitativamente, bem como a sua dinamicidade. No presente estudo foram analisados 30 olhos de 17 pacientes portadores de GCS e 30 olhos de 16 pacientes nao glaucomatosos nos exames previos (Prev), ou seja, sem a instilacao de drogas midriaticas e cicloplegicas, com colirio de tropicamida a 1% (C1) e com associacao de colirios de tropicamida a 1% mais cloridrato de fenilefrina a 10% (C2). A profundidade da camara anterior, o comprimento da iris, o toque iris-cristalino e a distancia iris-zonula dos olhos glaucomatosos e nao glaucomatosos foram estudados apos a instilacao desses colirios e analisados estatisticamente. Nos exames previos (Prev), nao houve diferenca estatisticamente significante da profundidade da regiao central da camara anterior dos olhos glaucomatosos e nao glaucomatosos, nem do comprimento da iris, do toque iris-cristalino e da distancia iris-zonula nas regioes 12, 3, 6 e 9 dos olhos estudados. Houve diferenca estatisticamente significante da profundidade da regiao central da camara anterior dos olhos glaucomatosos e nao glaucomatosos entre os exames previos (Prev), com colirio de tropicamida a 1% (C1), com associacao de colirios de tropicamida a 1% e cloridrato de fenilefrina a 10% (C2), exceto, destes entre si. Houve diferenca estatisticamente significante do comprimento da iris, do toque iris-cristalino e da distancia iris-zonula entre os exames previos (Prev), com colirio de tropicamida a 1% (C1), com associacao de colirios de tropicamida a 1% e cloridrato de fenilefrina a 10% (C2) e destes entre si, nas regioes 12, 3, 6 e 9 dos olhos glaucomatosos e nao glaucomatosos. Nao foi estatisticamente significante a diferenca do comprimento da iris, do toque iris-cristalino e da distancia iris-zonula entre as regioes 12, 3, 6 e 9 dos olhos glaucomatosos e nao glaucomatosos nos exames previos (Prev), com colirio de tropicamida a 1% (C1) e com associacao de colirios de tropicamida a 1% e cloridrato de fenilefrina a 10% (C2)
- ItemAcesso aberto (Open Access)Effect of mydriatic and cycloplegic drugs in glaucomatous and nonglaucomatous eyes using ultrasound biomicroscopy(Conselho Brasileiro de Oftalmologia, 2000-08-01) Miranda, Danielle [UNIFESP]; Allemann, Norma [UNIFESP]; Mello, Paulo Augusto de Arruda [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Purpose: To study the anatomical relationships of the anterior segment of the eye, using ultrasound biomicroscopy in patients with chronic simple glaucoma and nonglaucomatous eyes, after mydriatic and cycloplegic instillation. Patients and Methods: Thirty eyes with chronic simple glaucoma and 30 nonglaucomatous eyes were studied. Anterior chamber depth, iris-lens contact and iris-zonule distance were measured, in both groups, using ultrasound biomicroscopy, in the three following conditions: without mydriatic and cycloplegic instillation, with 1% tropicamide alone and with 1% tropicamide plus 10% phenylephrine chlorhydrate. Results: Ultrasound biomicroscopy examinations of glaucomatous and nonglaucomatous eyes, without mydriatic and cycloplegic instillation, showed no statistically significant difference in the central anterior chamber depth, iris-lens contact and iris-zonule distance. The difference between examinations without mydriatic and cycloplegic instillation, with 1% tropicamide and with 1% tropicamide plus 10% phenylephrine chlorhydrate was statistically significant regarding the variables measured in both groups. Conclusions: No anatomical difference was observed between the anterior segment of glaucomatous and nonglaucomatous eyes after mydriatic and cycloplegic instillation. The results of this clinical investigation, using ultrasound biomicroscopy were innovative, allowing a dynamic and quantitative evaluation of the anatomical relationships between intraocular structures when submitted to mydriasis and cycloplegy, which up to now were only qualitative.

[UNIFESP]; Prata Junior, João Antonio [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Purpose: To analyze the influence of cicloplegia on latanoprost ocular hypotensive effect in normal subjects. Patients, material and methods: The study included 17 normal volunteers without previous ocular diseases, trauma or surgeries. One drop of 0.005% latanoprost was given to both eyes and after 20 minutes one drop of 1% ciclopentolate was instilled only in right eyes. Intraocular pressure (IOP) was measured using Goldmann tonometry in both eyes at the following time points: before latanoprost, after 20 minutes, 1 hour and 20 minutes, 3 hours and 20 minutes and 4 hours and 20 minutes. IOP was compared between right and left eyes (Wilcoxon test) and among the different times of study for each eye (Friedman test). Percentual IOP change from initial reading was calculated and compared between OD and OS. Results: No statistical differences were detected regarding the initial, 20 minute, 1 hour and 20 minute and 3 hours and 20 minute measurements. For the 4 hours and 20 minute readings the comparison of IOP levels and their percentual change between OD and OS showed statistical differences. The mean IOP was lower in OD (IOPOD = 10.9 ± 2.2; IOPOS = 11.8 ± 2.2; p = 0.009). The right eyes showed statistically higher IOP percentual reduction than left eyes (delta%OD = -28.1±7.4; delta%OS = -21.6 ± 9.6; p = 0.006). Conclusion: The results showed a higher short-term latanoprost IOP reduction in eyes treated with ciclopento-late 1%, as compared to the contralateral eyes.
