It brings improvements in clinical parameters and reduces direct and indirect costs to health systems. However, pharmaceutical care service implementation is complex and difficult. In this scenario, however, there are experiences that seem to advance the effectiveness of Pharmaceutical Care, such as the pharmacy service of the Paulista Institute of Geriatrics and Gerontology (IPGG). Objective: Study how IPGG pharmaceutical care service implementation occurs. Materials and methods: This is a case study, constructed in quantitative and qualitative data sources, using document analysis, participant observation and interviews. Qualitative material content was analyzed thematically and quantitative data was described with central tendency measures. Results and Discussion: Three themes proved to be fundamental for the service implementation process: Context, Mechanisms for Pharmaceutical Care vibilization and Pharmaceutical Care consolidation. Pharmaceutical care service implementation context favored interdisciplinary relationships development and service systematization, since it is a training institution in continuing education of professionals. Adaptation of medications access strategic actions were necessary to enable the service and won team trust, opening space for pharmaceutical work. Conclusion: Pharmaceutical care service implementation process the in the IPGG was done in a strategic way. Contextual elements was used to construct a different pharmaceutical care model comparing the idealized one, it supplies the health needs and inserts the pharmacist in the production of the care, resulting in professional visibility and recognition of different roles in the team.
