- ItemAcesso aberto (Open Access)Consenso brasileiro sobre políticas públicas do álcool(Associação Brasileira de Psiquiatria - ABP, 2004-05-01) Laranjeira, Ronaldo [UNIFESP]; Romano, Marcos [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)This is the summary of a meeting where a group of experts, representing several health organizations and academic departements from different parts of Brazil, created a consensus about the main alcohol policies which should be implemented by different levels of the brazilian government. The World Health Organization has been suggesting for 30 years the actions that should be implemented for the public good. Two important conclusions were reached: 1) The research establishes beyond doubt that public health measures of proven effectiveness are available to serve the public good by reducing the widespread costs and pain related to alcohol use; 2) To that end, it is appropriate to deploy responses that influence both the total amount of alcohol consumed by a population and the high-risk contexts and drinking behaviours that are so often associated with alcohol-related problems. To conceive of these intrinsically complementary approaches as contractory alternatives would be a mistake. The objectives of the consensus are: 1) To make the scientific evidences more available to the Brazilian policy makers; 2) To facilitate the evaluation of the available estrategies according their effectivity, scientific support, cost and cultural adaptability; 3) To make the brazilian health professionais familiar with the priorities of alcohol policies.

Métodos: Estudo metodológico baseado no referencial de Spínola, referente à validação de conteúdo de um protocolo clínico que seguiu três etapas. Na etapa 1 (proposta do protocolo) construiu-se o instrumento de coleta de dados delineado em evidências da literatura científica e na experiência profissional dos pesquisadores, contendo 03 categorias, 09 tópicos, 31 itens e 106 subitens assistenciais. Na etapa 2 (validação de conteúdo) realizou-se a apreciação das variáveis da proposta do protocolo (primeira rodada de Delphi) por meio de ajuizamento com comitê de especialistas na temática. Posteriormente, realizou-se uma revisão integrativa da literatura referente as complicações no pós-operatório de transplante cardíaco composta por seis etapas e estratégia PICOT. A busca foi realizada em oito bases de dados (LILACS, MEDLINE®/PubMed®, Embase®, CINAHL, Cochrane Library®, Scopus®, Web of Science e Ovid) e para seleção dos artigos utilizou-se o programa Rayyan QCRI. Em seguida procedeu-se ao ajuizamento das variáveis do protocolo (segunda rodada de Delphi) com a avaliação por experts brasileiros e estrangeiros. Na etapa 3 realizou-se a construção das variáveis do protocolo clínico (constructo final). Este estudo foi aprovado pelo CEP UNIFESP CAAE: 18298919.6.0000.5505. Resultados: Na primeira rodada de Delphi realizou-se, por videoconferência, a análise das variáveis propostas no instrumento de coleta de dados pelo comitê de especialistas na temática, composto por 02 médicos e 02 enfermeiras, assim compôs-se de 03 categorias, 09 tópicos, 31 itens e 119 subitens. Na revisão integrativa da literatura foram incluídos 05 artigos referentes ao período pós-operatório imediato. Na segunda rodada de Delphi participaram 04 (44,4%) experts do Brasil, 04 (44,4%) da Espanha e 01 (11,1%) do Equador com prática clínica em pós-operatório de transplante cardíaco entre 2 e 30 anos. Os resultados computaram 105 (88,2%) subitens com índice de validade de conteúdo total (IVC-T) de 01 (100,0%) e demostrou que a maioria obteve o IVC > 0,80 (80,0%) estipulado para este estudo. Conclusão: O protocolo (constructo final) foi estruturado com 03 categorias, 09 tópicos, 31 itens e 119 subitens, visando ser um instrumento para garantir a segurança do paciente e a qualidade da assistência prestada articulada aos avanços tecnológicos.

The development of dietary guidelines next to the action planning aligned to culture and health problems related to food in each country are strategies to improve dietary intake and nutritional well-being of the population. The American food pyramid was one of the best known icons for promoting healthy eating and, since its proposition, other guides were constructed to incorporate advances in nutrition science and enhance the role of qualitative indicators of dietary intake for the prevention of chronic diseases. Nutrition education is a process that aims to help individuals and groups to adopt desirable eating behaviors as part of a healthy lifestyle. The search for languages that enhance the dialogue with the leaner is a strategy for meaningful learning, whereby the individual is invited to leadthe process. The increasing interest and access to information technology is an opportunity to promote healthy food choices. Digital language was used to house a Food Guide on mobile devices; the outcome effect of this tool, however, is not known on the eating behavior or the acceptance of its users. OBJECTIVE.To evaluate the Digital Food Guide (DFG) as a resource to promote healthy food choices. MATERIALS AND METHODS. Retrospective cohort study involving analysis of food intake and body weight was conducted with DFG users of both sexes and aged between 19 and 50 years. An Index was created to assess the Diet Quality, associated with the guide – DQI-DFG. Psychometric properties of the instrument were used to evaluate its reliability and validity. The users' perception regarding the usability of the tool, healthy choices incorporation and transition to appropriate weight was measured by a Likert scale. RESULTS.Among the 442 subjects in the sample, 31.45% showed improvement of the diet quality and 60% lost weight. The validation study of the DQI-DFG showed weak correlations between the components and the total energy of the diet (r = -0.16 to 0.09), the highest correlation (r = 0.515) occurred between the component sugars and sweets and total score; internal consistency showed an α= 0.36; answering to Likert scale, users indicatedsatisfaction of most assertives, with an average 3.10 points outof possible 4.0. CONCLUSION.The increased interest and access to technology for mobile devices is an opportunity to promote healthy food choices. The DFG helped in improving dietary patterns and the loss of excess weight. Although the internal consistency is similar to that observed in international indexes, the reliability of the instrument can be improved in order to provide greater certainty and to qualify interpretation of the data of food intake. The perception was positive for all three dimensions studied, which demonstrates an assertive technological construction.

Unfortunately health professionals in Brazil have great difficulty dealing with this condition due to the lack of standard care orientation. Objective: This clinical consensus by Academia Brasileira de Otorrinolaringologia Pediatrica (ABOPe) and Sociedade Brasileira de Pediatria (SBP) aims to generate national recommendations on the care concerning tracheostomized children. Methods: A group of experts experienced in pediatric tracheostomy (otorhinolaryngologists, intensive care pediatricians, endoscopists, and pediatric pulmonologists) were selected, taking into account the different regions of Brazil and following inclusion and exclusion criteria. Results: The results generated from this document were based on the agreement of the majority of participants regarding the indications, type of cannula, surgical techniques, care, and general guidelines and decannulation. Conclusion: These guidelines can be used as directives for a wide range of health professionals across the country that deal with tracheostomized children. Methods This study used the Delphi technique to consult school nutrition experts in Brazil. These experts were selected among the authors of articles published between 2010 and 2015 that were identified by searching the Web of Science database, using the keywords 'school feeding', 'school nutrition program', and 'school food program', as well as the authors of official documents on this topic. The Likert method was used to record respondent perceptions in two analytical dimensions: foods that are part of a healthy menu for school feeding, and composition of an indicator of nutritional quality for school feeding menus. Results Most respondents (n=27) were affiliated with public institutions (92.59%) and had over six years of experience in the area (70.36%). Assertions resulted in consensus according to the analysis criteria. A consensus was established for fresh food, fruits and vegetables, dairy products, beans, meat and eggs, and a schedule compatible with the meal as promoters of healthy eating, and processed foods, sweets, and fat excess as risk attributes. Conclusion Since a consensus was not reached in the first round, there is a need to broaden the debate on issues related to foods rich in sugar, replacement of meals by sweets, and foods rich in fat.

METHODS: Ten Latin American experts on ocular allergy participated in a 4-round Delphi panel approach. Four surveys were constructed and answered by panelists. A two-thirds majority was defined as consensus. Definition, classification, staging and diagnosis and treatment recommendations were the main outcomes. RESULTS: Ocular allergy was proposed as the general term to describe ocular allergic diseases. Consensus regarding classification was not reached. Signs and symptoms were considered extremely important for the diagnosis. It was consensus that a staging system should be proposed based on the disease severity. Environmental control, avoidance of allergens and the use of artificial tears were recommended as first line treatment. The secondary treatment should include topical anti-histamines, mast cell stabilizers and multi actions drugs. Topical non-steroidal anti-inflammatory and vasoconstrictors were not recommended. Topical corticosteroids were recommended as third line of treatment for the most severe keratoconjunctivitis. Consensus was not reached regarding the use of systemic corticosteroids or immunosuppressant. Surgical approach and unconventional treatments were not recommended as routine. CONCLUSION: The task of creating guidelines for ocular allergies showed to be very complex. Many controversial topics remain unsolved. A larger consensus including experts from different groups around the world may be needed to further improve the current recommendations for several aspects of ocular allergy.
