- ItemAcesso aberto (Open Access)Avaliação clínica e por imagem da cirurgia artroscópica em articulação temporomandibular(Universidade Federal de São Paulo (UNIFESP), 2016-08-23) Moreno, Roberto [UNIFESP]; Abrahão, Marcio [UNIFESP]; http://lattes.cnpq.br/4440305851848835; http://lattes.cnpq.br/3045207445564225; Universidade Federal de São Paulo (UNIFESP)Objective: To evaluate the efficacy of surgical treatment with arthroscopy , the dysfunctions of the temporomandibular joints ( TMD ) . Method : We evaluated 450 patients with temporomandibular joint dysfunction (TMD ) , treated for 3 months with miorelaxante and physiotherapy board, after three months , only 32 patients had no improvement as local pain in the temporomandibular joint (TMJ ) and the mouth open . The 32 patients underwent physical examination , subjective assessment of pain using the Visual Analogue Scale (VAS ) , MRI and performed the arthroscopic lysis and washing. The data were compared with the same group after six months of surgery. Result: Arthroscopic surgery for TMD is effective and satisfactory results with improvement of pain in ATM and mouth opening , with no improvement regarding the evaluation of images , with a low complication rate .
- ItemAcesso aberto (Open Access)Classificação psicoacústica do zumbido persistente(Universidade Federal de São Paulo (UNIFESP), 2016-10-25) Suzuki, Flavia Alencar de Barros [UNIFESP]; Penido, Norma de Oliveira [UNIFESP]; http://lattes.cnpq.br/7060786297081212; http://lattes.cnpq.br/8460049642723926; Universidade Federal de São Paulo (UNIFESP)Objective: To propose a new classification of persistent tinnitus using psychoacoustic characteristics and its association to the degree of discomfort, the impact on quality of life, the pitch, loudness and its masking. Method: A total of 110 adult patients of both genders treated at the Tinnitus Outpatient Clinic of UNIFESP according to the inclusion and exclusion criteria were included in the study. Patients underwent otorhinolaryngological, audiological, Pitch Matching and Loudness, Visual Analogue Scale (VAS), Tinnitus Handicap Inventory (THI) and Minimum Masking Level (MML) assessment. Results: A total of 181 complaints of tinnitus were identified in these 110 patients, being 93 (51%) Pure Tone (PT), and 88 (49%) Noise; 19 Low Pitch and 162 High Pitch; with a mean VAS score in PT of 5.47 and Noise of 6.66; mean PT Loudness of 12.31 dBSL and Noise of 10.51 dBSL. As for THI and MML, the 110 patients were separated into 3 groups with tinnitus, PT, Noise and multiple tinnitus (PT+Noise), with a mean THI being higher in the group with multiple tinnitus, of 61.38. For MML, we used White Noise (WN) and Narrow Band (NB) masking noise for high and low frequency. There were similarities between the groups with PT and multiple tinnitus. In the Noise group, different responses were found when using the NB at low frequency. Conclusion: In this psychoacoustic classification of tinnitus it was observed that: there is an association between the type of tinnitus perception and the sounds produced in the audiometer; the PT type showed higher loudness; the Noise type caused more discomfort; Multiple tinnitus is the one with a more severe effect on quality of life and less intensity is required for masking tinnitus when the pitch is similar to the masking noise used.
- ItemAcesso aberto (Open Access)Efetividade do tratamento da dor pós-operatória de pacientes submetidos a cirurgia abdominal de médio e grande porte em dois hospitais públicos de Brasília(Universidade Federal de São Paulo (UNIFESP), 2010-10-27) Gaudard, Ana Márcia Yunes Salles [UNIFESP]; Saconato, Humberto [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Introduction: Moderate to intense pain is prevalent after surgical procedures and it is associated with increases in postoperative morbidity and mortality. Objective: Evaluate the effectiveness of control and the factors associated with perception of pain intensity in patients undergoing abdominal surgery in hospital surgical clinics of two public hospitals in Brasilia Methods: Transversal and descriptive study, through document analysis, and interviews with patients, performed at two public hospitals from Brasilia. Data were collected about clinical features, surgical procedures and pain management from 342 patients. Pain was evaluated using the visual analog scale on the first 48 postoperative hours. The results were compared with data encountered on the bibliographic review. Results: 100% of patients in the prescription of analgesics postoperative was not based on the best scientific evidence. Moderate to intense pain was observed in 38.9% of patients. The most prescribed analgesic drug was dipyrone (83.6%). Of the patients using dipyrone alone, 29.2% of patients reported moderate to intense pain. Moderate to intense pain was significantly associated with women, with the use of anesthetic procedures other than epidural and therapeutic plans prescribed by the attending doctor. The majority of patients (50.9%) didn’t receive previous information about postoperative pain, 61% of patients who were in pain didn’t ask for relief and 80% of them had records of pain assessment, but without the use of scales and pain characterization. Conclusions: Postoperative pain management in the evaluated hospitals does not follow conducts based on the best evidence. The postoperative pain was more severe in female patients and in patients under treatment regimens prescribed by staff
