- ItemAcesso aberto (Open Access)A behavioral intervention in a cohort of Japanese-Brazilians at high cardiometabolic risk(Revista De Saude Publica, 2012-08-01) Almeida-Pititto, Bianca de; Griffin, Simon J.; Sharp, Stephen J.; Hirai, Amelia Toyomi [UNIFESP]; Gimeno, Suely Godoy Agostinho [UNIFESP]; Ferreira, Sandra Roberta Gouvea [UNIFESP]; Universidade de São Paulo (USP); MRC; Universidade Federal de São Paulo (UNIFESP)OBJECTIVE: To assess the effect of a health promotion program on cardiometabolic risk profile in Japanese-Brazilians.METHODS: A total of 466 subjects from a study on diabetes prevalence conducted in the city of Bauru, southeastern Brazil, in 2000 completed a 1-year intervention program (2005-2006) based on healthy diet counseling and physical activity. Changes in blood pressure and metabolic parameters in the 2005-2006 period were compared with annual changes in these same variables in the 2000-2005 period.RESULTS: During the intervention, there were greater annual reductions in mean (SD) waist circumference [-0.5(3.8) vs. 1.2(1.2) cm per year, p<0.001], systolic blood pressure [-4.6(17.9) vs. 1.8(4.3) mmHg per year, p<0.001], 2-hour plasma glucose [-1.2(2.1) vs. -0.2(0.6) mmol/L per year, p<0.001], LDL-cholesterol [-0.3(0.9) vs. -0.1(0.2) mmol/L per year, p<0.001] and Framingham coronary heart disease risk score [-0.25(3.03) vs. 0.11(0.66) per year, p=0.02] but not in triglycerides [ 0.2(1.6) vs. 0.1(0.42) mmol/L per year, p<0.001], and fasting insulin level [ 1.2(5.8) vs. -0.7(2.2) IU/mL per year, p<0.001] compared with the pre-intervention period. Significant reductions in the prevalence of impaired fasting glucose/impaired glucose tolerance and diabetes were seen during the intervention (from 58.4% to 35.4%, p<0.001; and from 30.1% to 21.7%, p=0.004, respectively).CONCLUSIONS: A one-year community-based health promotion program brings cardiometabolic benefits in a high-risk population of Japanese-Brazilians.

However, reflections on the planning of educational actions, its possibilities and results are scarce, especially in the sports environment, considered a scenario of vulnerability for adopting inappropriate food practices. The objectives were: to describe and analyze the effect of a Food and Nutrition Education project developed with a rhythmic gymnastics team; to verify that the strategies adopted to the Arch of Maguerez, chosen as a theoretical reference and to identify the perceptions of the athletes and technical commission on the effectiveness of the project. Participants and methods: The study was divided into 3 stages: 1) Diagnostic evaluation of 14 rhythmic gymnasts: anthropometric evaluation and body composition; knowledge assessment nutritional; dietary assessment; behavior change intention stage to feed; satisfaction with body image; interviews; 2) Elaboration and application of the project from the diagnostic stage for eight months; 3) Reapplication of the instruments used in the first stage, including interview with the technical committee. Results: The mean age of the athletes was 18.1 ± 2.3 (min = 15.0; max = 22.0) with 9.4 ± 3.5 (min = 2.0, max = 14.0) years of sports practice. After EAN actions, it was possible to observe an increase in the percentage of nutritional knowledge questionnaire (p = 0.012); increased milk intake (p = 0.01) and fish (p = 0.02); reduction of frying intake (p = 0.02) and meat fatty acids (p = 0.03). EAN actions were able to influence positively choices, arousing motivation and interest in worked. Athletes and coaches report specific changes in diet related to the themes worked out during the EAN project. In addition, athletes were able to identify that the project planning was based on the diagnosis, and the themes are related to the reality that live. The strategies used in the activities were also adequate, facilitating the attention and the learning of the athletes.

This is a cross-sectional, descriptive study of qualitative and quantitative nature. Perceptions, concepts and practices of FNE of teachers (n=56) and coordinators (n=3) of two public elementary city schools of São Paulo (EMEFs), and nutritionists (n=28) of the School Feeding Department linked to the National School Feeding Programme were investigated using a Likert-type attitudinal instrument, narratives and semi-structured interviews, respectively. The Likert-type attitudinal instrument was built based on four dimensions and its validity was verified by linear correlation and reliability by applying the test of Wilcoxon signed rank. Results were described in mean values by assertions. Interviews and narratives were transcribed in verbatim and after that analyzed by using the content analysis method, from thematic nuclei. Categories and emerged from context and registry units. The study is in line with the ethical principles of research and authorization was granted by the Research Ethics Committee of the Federal University of São Paulo, under protocol number 1,242,853 of 09/24/2015. For teachers FNE actions must involve other professionals and areas of knowledge and should include in their conceptual base, the Nutrition and Food Security and the Human Right for an Adequate Nutrition themes. For coordinators FNE represents mainly the supply of food and its nutritional aspects. Nutritionists perceive the issue in different ways, especially FNE as a process that involves awareness raising and knowledge transfer aiming to achieve an eating behavior change. In the perception of teachers, coordinators, and nutritionists it can be observed the coexistence of different pedagogical trends, highlighting the traditional, as the more privileged models, as well as progressive models. These actors emphasized the need to develop interprofessional practices in FNE, and they consider important the presence of other areas of knowledge to guide these practices in school, as well as the need for permanent processes involving interdisciplinary and cross-content approaches.
