METHODS: Performance observations were carried out among 40 elderly patients and their respective caregivers were interviewed. During home visits, data on caregiver's perception were collected by an independent observer using the Functional Independence Measure (FIM) instrument. Health providers evaluated elderly patients on their functional capacity in six dimensions: self-care, sphincter control, mobility, walking, communication and social cognitive skills. RESULTS: Intraclass correlation coefficient (ICC) showed an excellent agreement in all dimensions and the highest total FIM score was 0.95. Only the dimensions self-care and mobility showed a bias error between measures (signal test: p<0.001). Bias errors were not found when Bland-Altman graphic was applied. CONCLUSIONS: The agreement between health provider's and caregiver's measures demonstrated a good interaction between program team and caregivers, which is of great importance on evaluating and treating elderly patients. The results also suggest that when a valid and reliable instrument such as FIM is used, there is a high reproducibility of measures, even for different types of evaluations.
- ItemAcesso aberto (Open Access)Exercício resistido em idosos frágeis: uma revisão da literatura(Pontifícia Universidade Católica do Paraná, 2012-06-01) Câmara, Lucas Caseri; Bastos, Carina Corrêa; Volpe, Esther Fernandes Tinoco [UNIFESP]; Universidade de São Paulo (USP); Universidade Católica do Rio Grande do Sul; Universidade Federal de São Paulo (UNIFESP)INTRODUCTION: Fragility syndrome, very common in elderly people, consists of a set of signals and symptoms in which is present criteria such as not intentional weight loss (approximately 5%) in a year, reduction in the walking speed, low physical activity levels, subjective exhaustion and muscular strength reduction. The increasing effect of these changes related to age, which include sarcopenia, immunity functional disorder and neuroendocrinous misconduct, increase the vulnerability of the organism to stress, reducing the ability to adapt, compensate or modulate these stimuli. Several intervention proposals have been made to attenuate this process, and resistance exercises (RE) was one of the options studied. OBJECTIVES: To evaluate the effects of RE on the physiopathology of fragility syndrome. MATERIALS AND METHODS: A bibliographic review of the period 2004-2010 was made based on the data of LILACS, MEDLINE and PubMed. Results: Changes in the hormonal and immune systems were observed acting in a systemic way by reverting or minimizing the effects of sarcopenia. CONCLUSION: Resistance exercises should serve as therapy to those elderly who are fragile and do not present any health problems.

The methods used were a six-month observation clinical study. The following tools were used to assess fourteen elderly patients over 80 years old at admission at the day care and for six months thereafter: Brazilian OARS Multidimensional Functional Assessment Questionnaire (daily life activities), Mini-Mental State Exam (cognition), Berg Balance Scale (functional balance evaluation) and Timed Up and Go Test (mobility). Data were analyzed using descriptive statistics and Wilcoxon and paired-samples T tests to verify statistical differences over time, a=0.05. The results showed significant differences between assessment and reassessment of the global functional capacity scores (p=0.019) and postural transfer balance-related ability (p=0.041), showing improvement over time. After follow-up, scores related to the other variables remained unchanged. Conclusion: multidisciplinary health care at the geriatric day care center reinforced improvements or maintenance of the participants functional capacities, proving to be a promising alternative health care mode for the elderly.
