- ItemSomente MetadadadosApproaches to reduce the glycemic response of gluten-free products: in vivo and in vitro studies(Royal soc chemistry, 2016) Capriles, Vanessa Dias [UNIFESP]; Arêas, José Alfredo Gomes; Universidade Federal de São Paulo (UNIFESP)This article aims to discuss the current approaches used to reduce the postprandial glycemic response (GR) of starchy gluten-free products (GFP) and to provide some further prospects. The GR is evaluated through in vivo trials to determine the glycemic index (GI) and the glycemic load (GL) of foods or using the in vitro starch digestibility method to predict the GI. The reviewed studies demonstrated that a reduction in the GR of GFP can be achieved by replacing traditionally used refined flours and starches with nutrient-dense alternative raw materials due to an increase in dietary fiber, resistant starch and protein contents, which can alter the rate of starch digestion, reduce the GI and dilute the amount of available carbohydrate; both the factors contribute to reduce the GL. The addition of viscous dietary fibers could also be a successful way to reduce the GR of GFP. Some studies have demonstrated the potential of germination of GF grains and sourdough fermentation in decreasing the GR of GF bread. Further studies could evaluate alternative GF flours, soluble fibers, resistant starch, and other formulation factors that may affect the GR of different types of GFP. New studies should be performed to test and optimize processing methods by considering the reduction of GR of GFP. Another key aspect for further investigation is the confirmation of the promising results obtained through in vitro assays by in vivo GI evaluation.

The development of dietary guidelines next to the action planning aligned to culture and health problems related to food in each country are strategies to improve dietary intake and nutritional well-being of the population. The American food pyramid was one of the best known icons for promoting healthy eating and, since its proposition, other guides were constructed to incorporate advances in nutrition science and enhance the role of qualitative indicators of dietary intake for the prevention of chronic diseases. Nutrition education is a process that aims to help individuals and groups to adopt desirable eating behaviors as part of a healthy lifestyle. The search for languages that enhance the dialogue with the leaner is a strategy for meaningful learning, whereby the individual is invited to leadthe process. The increasing interest and access to information technology is an opportunity to promote healthy food choices. Digital language was used to house a Food Guide on mobile devices; the outcome effect of this tool, however, is not known on the eating behavior or the acceptance of its users. OBJECTIVE.To evaluate the Digital Food Guide (DFG) as a resource to promote healthy food choices. MATERIALS AND METHODS. Retrospective cohort study involving analysis of food intake and body weight was conducted with DFG users of both sexes and aged between 19 and 50 years. An Index was created to assess the Diet Quality, associated with the guide – DQI-DFG. Psychometric properties of the instrument were used to evaluate its reliability and validity. The users' perception regarding the usability of the tool, healthy choices incorporation and transition to appropriate weight was measured by a Likert scale. RESULTS.Among the 442 subjects in the sample, 31.45% showed improvement of the diet quality and 60% lost weight. The validation study of the DQI-DFG showed weak correlations between the components and the total energy of the diet (r = -0.16 to 0.09), the highest correlation (r = 0.515) occurred between the component sugars and sweets and total score; internal consistency showed an α= 0.36; answering to Likert scale, users indicatedsatisfaction of most assertives, with an average 3.10 points outof possible 4.0. CONCLUSION.The increased interest and access to technology for mobile devices is an opportunity to promote healthy food choices. The DFG helped in improving dietary patterns and the loss of excess weight. Although the internal consistency is similar to that observed in international indexes, the reliability of the instrument can be improved in order to provide greater certainty and to qualify interpretation of the data of food intake. The perception was positive for all three dimensions studied, which demonstrates an assertive technological construction.

- ItemSomente MetadadadosEszopiclone versus zopiclone in the treatment of insomnia(Hospital clinicas, univ sao paulo, 2016) Pinto Júnior, Luciano Ribeiro [UNIFESP]; Bittencourt, Lia Rita Azeredo [UNIFESP]; Treptow, Erika Cristine [UNIFESP]; Braga, Luciano Rotella [UNIFESP]; Tufik, Sergio [UNIFESP]OBJECTIVE: To determine the therapeutic effects of two selective GABA-A agonists, zopiclone and eszopiclone, in the treatment of insomnia. METHODS: This study comprised a phase III, single-center, randomized, double-blind, double-dummy, parallel-group, non-inferiority trial. Patients were randomized to receive zopiclone 7.5 mg or eszopiclone 3 mg, both orally, for four weeks. In total, 199 patients were evaluated during two visits and then followed for at least six weeks. The primary endpoint was the Insomnia Severity Index after four weeks of treatment. Secondary endpoints were obtained through polysomnography data, including total sleep time, sleep latency and sleep efficiency. The frequency of adverse events was also analyzed. ClinicalTrials.gov: NCT01100164. RESULTS: The primary efficacy analysis demonstrated the non-inferiority of eszopiclone over zopiclone. Analysis of objective parameters assessed by polysomnography showed that eszopiclone increased total sleep time and also improved sleep efficiency. The safety profile of both study treatments was similar and the most common events reported in both groups were dysgeusia, headache, dizziness, irritability and nausea. Adverse events were observed in 223 patients, 109 (85.2%) in the eszopiclone group and 114 (87.7%) in the zopiclone group. CONCLUSION: Based on the Insomnia Severity Index at the end of four weeks of treatment, eszopiclone demonstrated efficacy comparable to that of zopiclone in the treatment of insomnia, increasing total sleep time as well as sleep efficiency according to polysomnography.

[UNIFESP]; Pedrazzoli, M.The objective of this study was to evaluate the alterations in sleep and circadian parameters during the aging process. The study sample comprises volunteers older than 18 up to 90 years of age that answered the Pittsburgh Sleep Quality Index (PSQI) and the Horne and Ostberg circadian preference questionnaire. We observed that the shift to morningness with increasing age is associated with a significant worsening in sleep quality. We discuss that this sleep profile characterized by morningness and worse sleep quality observed in elderly, when compared to younger people, reflects not necessarily a pathological state, but an expected profile for this age group.
