This trend tends to worsen with the aging of the population which will demand more complex care and for a longer period, that due both scientific and technological evolution. In line with today's challenges, clinical computing can be defined as the study and use of data and healthcare information technology to improve performance and ability to maintain quality in healthcare. Methods: This is an experimental study with allocation of a known population of individuals (inpatients). Data were collected from both Adverse Event Management System and Electronic Health Records System. Data were collected from a 24-month period with a total sample of 106,658 patient records. Results: The use of the decision support system evaluated in this study has contributed to reduce cases of adverse events related to pressure ulcer developed in the inpatient unit. There was a 16.2% reduction in total cases, reduction of 42.9% in cases of Pressure Ulcer developed in the inpatient unit by patients over 60 years old and reduction of 46.5% in cases of complex lesions (stages 3, 4 and deep tissue injury) developed by patients over 60 years old. No contributions from the decision support system to reduce cases of fall-related adverse events were identified. Discussion: The study shows that the fact that activating decision support systems and providing this support in the form of alerts or protocol suggestions does not mean that positive results will be obtained. It is necessary to adopt change management methodologies, validations of interfaces with professionals who know the workflows and attention to data quality so that the information generated is reliable. Conclusions: This study demonstrated that integrated decision support systems, considering the large amount of data inside Electronic Health Record systems, can add important contribution to healthcare services for inpatients.
- ItemAcesso aberto (Open Access)Livro sobre simulação realística na prevenção de lesão por pressão(Universidade Federal de São Paulo (UNIFESP), 2020-11-27) Marques, Eder Alves [UNIFESP]; Blanes, Leila [UNIFESP]; Universidade Federal de São PauloIntroduction: The pressure injury (LP) causes tissue damage in certain areas of the body and prevention is the best way to avoid this damage. Healthcare professionals are responsible for preventing pressure injuries. An active methodology with realistic simulation provides influence and change in the healthcare team’s learning process. Objective: To develop a book on realistic simulation in the prevention of pressure injuries. Method: Descriptive-exploratory study of the development of a book on realistic simulation in the prevention of pressure injuries. Approved by the Committee of Ethics in Research of the Federal University of São Paulo (UNIFESP) according to CAAE number 28946619.0.0000.5505. A search for priority was carried out, as well as bibliographic survey, content structuring and validation by expert judges using the Delphi Technique. Results: In the validation, the global Content Validity Index was 0.86 in the first round and 1.0 in the second Delphi round. The book has 78 pages, six chapters and 25 figures that complement the text. Conclusion: A book on realistic simulation in the prevention of pressure injuries was developed and validated.
The pressure injuries result from the continuous pressure exertec by bony pcoeminences in the tegument pressed against a surface. Apparatus and devices have beem proposed in na attempt do mitigate the pressure in these áreas, mosf of which are highly complex and costly, which makes it difficult to adhere to treatment, especially after hospital discharge. Objective: To perform a literature review and search for anteriority on meshes or clothing to prevent pressure injuires in the regions of bony proeminences of the hip. Method: The search covered from April 1988 to July 2020. It was carried on the databases (Cochrane Library, Lilacs, Scielo, Medline, Web of Science & SSOPUS), having as inclusion criteria clothing aimed at preventing pressure injuries in the region of bone proeminences of the hip; there was no criteria of non-inclusion. The search of anteriority was made in the patent databases: LATIPAT (Espace Patent Search), Google Patent Search, USPTO (United States Patent and Tradmark Office), WiPO (Wordl Intellectual Property Organization), JPO (Japan Property Office, CNIPA (National Intellectual Property Agency – China) and INPI – e-patents (National Intellectual Property Institute. Results: two garments were found in the patent bases. Conclusion: No results were found for searches with the Keywords “prevention”, “injury”, “pressure”, “clothing”, “mesh” and related terms in the scientific literature. Two garments were found in the patent bases.
- ItemAcesso aberto (Open Access)www.previnalesao.com.br: website com orientações para cuidadores sobre prevenção de lesões por pressão em idosos acamados em domicílio(Universidade Federal de São Paulo (UNIFESP), 2018-02-15) Silva, Ana Paula Teles Cameli Rodrigues Da [UNIFESP]; Blanes, Leila [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Introduction: With the increase in life expectancy, a higher number of chronical conditions emerge. Some diseases can cause skin changes, leaving elderly people more vulnerable to the development of Pressure Lesions (PL). In this sense, it is necessary to direct care and attention to the bedridden elderly to prevent PL. Objective: Develop and validate a website with orientation for caregivers on the prevention of pressure lesions in elderly bedridden at home. Methods: Study for the development and validation of a website for orientation regarding elderly bedridden at home. The study was approved by the Ethics in Research Committee of UNIFESP, no 1343/20106 and CAAE 60565616.3.0000.5505. Literature search was conducted using the following databases: Scielo, Pubmed and Google Scholar research engine. No website was found with this intent. Subsequently, the website was developed. The bibliographic content was done on the Scielo and Pubmed databases and Google Scholar research engine, between the years of 2007 and 2017. The searched themes was elderly skin, definition, risk factors, prevention, support surface, incontinence and caregivers. Scientific articles, books and international consensus were used to develop the content of the website. A domain was acquired and registered in the Dot BR Information and Coordination Center. The WordPress platform was used to construct and manage the content of the website. Validation was done in two stages. In the first stage, the Delphi Method was used, applying an evaluation instrument on which the Content Validity Index was used. In the second stage, the target public was consulted to verify the opinion of caregivers concerning the organization of the website, writing style, appearance and motive. The Likert Scale was used to measure the percentage of agreement on the aspects of the instrument and its items. Results: The website was denominated www.previnalesao.com.br, using a language for easy understanding for the caregivers of the elderly. It presents none topics for the orientation of concepts, risk factors and prevention of PL. Using the Content Validity Index (CVI), validation obtained 90% of approval. Conclusion: A website was developed with orientations on the prevention of PL for the caregivers of elderly patients bedridden at home (www.previnalesao.com.br).
