Several clinical complications occur as a result of inadequate control of diabetes mellitus type II (DM2). There is evidence that patients have access to appropriate tools that promote the involvement, empowerment and active management of the disease perform better, both in their health status as in medical condition. Objectives: Develop and evaluate a prototype of personal health record application to patients affected by diabetes mellitus type II (DM2). Methods: For the application development it conducted a survey focused requirements in pathology with health professionals and also analyzed existing solutions, including existing literature reviews on PubMed. After the development, the application was available for five adult patients with diabetes mellitus type II (DM2), aged between 40 and 70 years, selected based on the objective of the study that is to evaluate the application content. The patients used the prototype for two weeks and have issued opinions through an evaluation questionnaire. Results: The application was developed for patients affected by diabetes mellitus type II to provide support in the registration data and information about the disease. In the assessment, the application appeared to be adequate for use. For application quality, it was observed that the kappa value was 0.302 in the patients analysis, indicating no correlation in the responses. However, regarding the distribution of the scores reported by the patients, it noted that the quality of software contents is good and adequate and that small improvements could be made in the content. Conclusion: The prototype of personal health record for patients affected by Diabetes Mellitus Type II is appropriate and functional to provide support in the control of data records and information about the disease.
