METHODS: A retrospective descriptive study was used to conduct the study. Four hundred and twenty four medical records of patients from medical and surgical units were reviewed from November 2006 to January 2007. The medical records were from patients who have been discharged from the hospital (56.1%) or those who have expired (43.9%). The focus of the review was on the demographic and background information, operation room flow sheet, nursing progress notes, nursing diagnoses, nursing orders, implementation of the nursing orders, medical orders, nursing documentation, discharge documentation, and documentation of death. RESULTS: The majority of nursing documentation was acceptable (64.7%). Only 8.7% of nursing documentation was of good quality. The remainder of nursing documentation was poor (26.7%). It is important to note that was difficult to measure nursing care outcomes reflected in nursing documentation on the medical records. This may affect patient safety and quality care. CONCLUSION: The findings of this study suggested deviation from recommended standards of nursing practice. They served to propose new goals and strategies to improve nursing documentation and the delivery of nursing care.
- ItemAcesso aberto (Open Access)Cross-cultural translation of quality instruments in the organ donation process(Escola Paulista de Enfermagem, Universidade Federal de São Paulo (UNIFESP), 2014-01-01) Knihs, Neide Da Silva; Schirmer, Janine [UNIFESP]; Roza, Bartira de Aguiar [UNIFESP]; Hospital Santa Isabel; Universidade Federal de São Paulo (UNIFESP)OBJECTIVE:To carry out the cross-cultural translation of quality instruments in the organ donation process from the National Transplantation Organization of Spain.METHODS:Methodological research with quantitative approach conducted in three Neurology reference hospitals in the South of the country. The instruments validation was carried out by mean of the steps of translation, backtranslation, synthesis and expert committee. The reliability was conducted by means of inter-observer equivalence and test and retest stability.RESULTS:There was a 100% match among all the judges in 89% of the items in the cross-cultural translation (11% of the items were not equivalent). In the reliability analysis, the Kappa coefficients corresponded to 0.86 and 0.95.CONCLUSION:The versions of the instruments were considered valid and reliable for use by institutions that carry out the organ donation process in Brazil.
- ItemAcesso aberto (Open Access)Oncoaudit: desenvolvimento e avaliação de aplicativo para enfermeiros auditores(Escola Paulista de Enfermagem, Universidade Federal de São Paulo (UNIFESP), 2014-04-01) Grossi, Luciane Mandia; Pisa, Ivan Torres [UNIFESP]; Marin, Heimar de Fatima [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Objective:To develop a web and mobile device application to search for chemotherapy drugs to support nursing audits of hospital bills and to evaluate user satisfaction and tool usability.Methods:Research of technological production for development of an application for web and mobile technology. The product was evaluated by nurse auditors using the System Usability Scale questionnaire. It was also evalutated by health informactics professionals using Nielsen's heuristics.Results: The application is available at http://telemedicina6.unifesp.br/projeto/oncoaudit. The mobile version can be is accessed at http://play.google.com/intl/pt-BR/about/index.html. Nurse evalaution indicated that the web and mobile versions addressed user needs. In the usability evaluation, 14 problems were identified in the mobile version and eight in the web system. Implementation of improvements according to the evaluation findings were made in both versions.Conclusion: The methods for development and evaluation were adequate to achieve the proposed objective.
