It has a fundamental importance for the establishment of a good doctor-patient relationship. The purpose of this research is to analyze the learning of empathy in undergraduate students of medical courses at public universities in the Northeast of Brazil, which are included in the last year of the medical internship. Therefore, we sought to understand the empathy conceptions of these students, knowing how the learning process for an empathic attitude during medical training occurred and identifying the way to improve this learning. The analytical-descriptive methodology was chosen, assuming the importance of the complementarity of the quantitative and qualitative approaches, in the best understanding of the object under study. At first, the Jefferson Scale of Medical Empathy - JSPE-S - student version was used. It is a self application instrument that evaluates the degree of empathy in clinical contexts and has been applied to 229 students. At a second moment – in a qualitative basis- focus groups were used with students, whose speeches were submitted to a thematic analysis, trying to understand the conceptions and learning of empathy in the undergraduate and making suggestions for the improvement of its teaching / learning. The results showed that the researched students present high averages related to the Perspective Taking and Compassionate Care that correspond to the first two factors of the JSPE-S. Regarding the third factor of scale named Standing in the Patient’s Shoes scale, they present a lower average and show difficulties when they are taking care of the patient. They do not recognize or identify relevant teaching moments during training in learning empathy, and when these items are mentioned, they relate more to theory than to medical practice. They emphasize that the methodology used is not very stimulating and that teachers lack training. They also demand an institutional look at their mental health. There were no significant differences between the sex and the age of the students regarding the empathic disposition. We concluded that this research confirmed the initial assumption: that medical graduation, in general, has not adequately prepared students for the practice of empathy in the physician-patient relationship. In proposing suggestions for enhancing empathy learning, they signal that it is important to include humanization as an integral part of the institutional culture.

Therefore, the following study was aimed at evaluating the empathy 3rd to 5th year medical residents from the Federal University of São Paulo to verify any possible differences between the empathy of physicians and surgeons and their relationship with the professional profile.
