Although plastic surgery has a bright history of innovation, the recent search for new therapies and methods in this specialty and more recently in regenerative medicine has heated up the market for a continuous search for innovations and technologies for the use of these cells of the SVF in an ethical, efficient way and easy to reproduce. Objective: to carry out a narrative review of the literature on plastic surgery procedures that use fat as a product, which involve obtaining ADSC and its potential for technological innovation in this processing. Method: a narrative literature review was carried out using the works published from January 1993 to August 2020, in English, Portuguese and Spanish, in the MEDLINE/PubMed databases. After the selection, exploratory reading, selective reading, material selection, text analysis, interpretive reading and writing were performed. Results: Of the 33 articles selected for the review, it was found that 23 articles used the enzymatic dissociation method to isolate stem cells derived from adipose tissue (ADSC), 25 articles reported that the plastic surgery procedure used was liposuction and 23 articles reported innovation technology using 3D models. Conclusions: The most used method for the isolation of ADSC was enzymatic dissociation, liposuction the most practiced procedure in plastic surgery and 3D models the most found innovation technology.

METHODS: Terms related to SMAS were selected from original articles retrieved from the ISI Web of Science and MEDLINE (PubMed) databases. Groups of terms were created to define a search strategy with high-sensitivity and restricted to scientific periodicals devoted to plastic surgery. This study included articles between January 1996 and May 2009, whose titles, abstracts, and keywords were searched for SMAS-related terms and all occurrences were recorded. RESULTS: A total of 126 original articles were retrieved from the main periodicals related to plastic surgery in the referred databases. Of these articles, 51.6% had SMAS-related terms in the abstract only, and 25.4% had SMAS-related terms in both the title and abstract. The term 'superficial musculoaponeurotic system' was present as a keyword in 19.8% of the articles. The most frequent terms were 'SMAS' (71.4%) and superficial musculoaponeurotic system (62.7%). CONCLUSION: The term SMAS refers to a structure relevant enough to start a discussion about indexing it as a keyword and as an official term in Terminologia Anatomica: International Anatomical Terminology.

Nicotine has a detrimental effect on cutaneous flap survival; although there are no experimental studies proving this effect on musculocutaneous flaps. The aim of this study is to verify the effect of nicotine on the rat TRAM flap. METHODS: 30 Wistar EPM-1 rats were randomly distributed in two groups: control and experimental. The animals of the control group received saline solution injected subcutaneously, in a volume of 0.2 ml, twice a day, during 28 days in the preoperative period. The animals of the experimental group were treated with nicotine, injected subcutaneously, in a dose of 2 mg/kg twice a day, during 28 days in the preoperative period. All the animals were submitted to the caudally based, right unipedicled TRAM flap. 48 hours after the procedure, a study of the viable area of the flap was done through photographic documentation. Statistical analysis was performed with nonparametric Mann-Whitney's test. RESULTS: The experimental group had significantly greater area of necrosis when compared with the control group (p<0.001). CONCLUSION: Nicotine increased the area of necrosis of the TRAM flap, in rats.

METHODS: One hundred thirty eight breast cancer patients who had undergone conservative treatment with oncoplastic approach (n=36) or not (control group, n=102), in the period from 2005 to 2010, were enrolled. Sociodemographic and clinical data were recorded. The direct costs of the surgical procedure were obtained and analyzed. RESULTS: Groups did not differ in regard to age (p=0.963), and patients in oncoplastic group had a longer time of hospital stay (p=0.000). The median direct cost for the oncoplastic group was R$461.00 and for the control group was R$229.00 (p=0.000). CONCLUSION: The oncoplastic approach has generated higher direct costs in conservative surgical treatment of breast cancer to SUS.

The purpose of this study is to evaluate the accuracy of ultrasonography to measure the width of rectus abdominis muscle diastasis. METHODS: Rectus diastasis was measured by ultrasonography preoperatively in 20 females. Rectus diastasis was measured in seven levels along the anterior rectus sheath by ultrasound after sustained deep inspiration and after expiration. Rectus diastasis, at the same levels, was also measured after its exposition during abdominoplasty by two independent observers, using a surgical compass. These values were compared using Wilcoxon's statistical text, for non independent values. RESULTS: There was no significant difference between the values obtained by ultrasound and those measured during surgery in the supra-umbilical levels and at the level of the umbilicus. However, below the umbilicus these values differed significantly, showing smaller values in the imaging evaluation. CONCLUSION: Ultrasonography is an accurate method to measure rectus diastasis above the umbilicus and at the umbilical level.
- ItemSomente MetadadadosUso do adesivo tecidual de fibrina derivado de veneno de serpente e avaliação da técnica de autoenxertia utilizando a pele do sulco nasogeniano(Universidade Federal de São Paulo (UNIFESP), 1998) Stolf, Hamilton Ometto [UNIFESP]; Dillon, Neuza Lima [UNIFESP]O adesivo de fibrina derivado de veneno de serpente mimetiza as reacoes finais do processo da coagulacao. O fibrinogenio de bufalo ao interagir com a fracao trombina-simile extraida do veneno de serpente libera uma fibrina monomerica que na presenca do fator XIII e do calcio torna-se polimerica, a qual age como cola biologica. A acao dessa cola permite a uniao das bordas de feridas e auxilia na hemostasia. No presente trabalho foi avaliada a capacidade de sintese e acao hemostatica desse adesivo na cirurgia cutanea, comparando-a com a sutura com fio monofilamentoso 5-0 e a tecnica utilizada para reparacao de tumores. Foram estudados 21 pacientes caucasianos, portadores de tumor cutaneo na regiao nasal, com idade media de 64 anos de ambos os sexos. Apos a retirada do tumor foi feito o reparo do defeito nasal pelo metodo de autoenxertia, sendo que a pele doadora provinha dos sulcos nasogenianos direito e esquerdo. Para a sintese cutanea do sulco nasogeniano direito utilizou-se o adesivo de fibrina; do esquerdo a sutura. O estudo comparativo das areas doadoras no mesmo paciente, 48 horas apos a cirurgia, mostrou maior eritema e edema no lado sutura e deiscencia em 28,6 por cento no lado cola (teste de Wilcoxon). No setimo pos-operatorio, houve maior eritema e mais irregularidade de contorno da incisao no lado sutura; enquanto no lado cola 33,3 por cento de deiscencia (teste de Wilcoxon). A avaliacao tardia do aspecto geral da cicatriz foi excelente no lado cola e bom no lado sutura. Nao foi detectado nenhum efeito toxico em nivel local ou sistemico. Verificou-se, portanto, que o adesivo de fibrina derivada do veneno de serpente teve capacidade adesiva total em 66,7 por cento e parcial em 33,3 por cento dos pacientes, constituindo-se em um metodo alternativo valioso na cirurgia cutanea

The post-bariatric patient has distinct clinical and laboratory characteristics, resulting in greater attention from plastic surgeons to provide quality preoperative planning. Objective: To develop a digital platform (website) for planning body contour surgery in post-bariatric patients. Method: The Design Thinking methodology was used and followed the four stages of Double Diamond: I. “Discover”: Desk research through the search of the literature covering the period from January 2010 to June 2020 in the databases. And Internet sites. Closed questionnaires were conducted based on the literature and sent online to plastic surgeons, II. “Define” in association with an Information Technology professional. III. “Develop” its content and layout and, finally, IV. "Deliver" the website. Results: The themes and content for creating the website were based on the results of searches on databases, research sites and responses to the questionnaires sent to experienced surgeons. It was defined that the product of the present study could: guide plastic surgeons in preparing post-bariatric patients to undergo body contouring procedures; prepare a complete and easy-to-read content to optimize comprehensive care for post-bariatric patients; present specific categorizations named as: Initial assessment (anamnesis, physical examination and photographic documentation); Nutritional assessment; Skeletal evaluation; Psychological assessment, and; General recommendations Conclusion: a website was developed to assist plastic surgeons in scheduling surgeries for body contouring in post-bariatric patients, available at www.planejamentoposbariatrico.com
