[UNIFESP]; Fed Univ Maranhao UFMA; Universidade Federal de São Paulo (UNIFESP)Duplication of the renal collecting system is the commonest major congenital malformation of the urinary tract, with an incidence of 1% among live births. Antenatal diagnosing of renal duplication and an associated ureterocele is infrequent. We report four cases of prenatally diagnosed unilateral duplication of the renal collecting system. In two of them, the renal duplication was associated with an ectopic ureterocele.

It can be diagnosed at any age range. Normally, drainage is achieved by 2 ureters, and there are only 5 reports in the literature of cake kidney drained by a single ureter. The authors describe one more case of this rare malformation of the urinary tract.

METHODS: This study was a literature review of articles published in the main health care databases from 1995 to 2005. RESULTS: The retrieved publications were organized into five themes: 1) exposition to risk factors for congenital defects, 2) use of technology to provide care to children with congenital defects, 3) evaluation of the delivery of care to children with congenital defects and their family, 4) nurse's competence to provide quality care to children with congenital defects, and 5) reports of lived experiences of professional nurses who provide care to children with congenital defects, and lived experience of children with congenital defects and their family. CONCLUSION: Nurses who provide care to children with congenital defects base their practice on strong theoretical and practical foundations; and, evidence based nursing interventions to provide the most efficacious care to children with congenital defects and their family continues to evolve.

B.; Koren, G.; Hosp Sick Children; Univ Fed Rio Grande Sul; Clin Hosp; Universidade Federal do Rio de Janeiro (UFRJ); Universidade Federal de São Paulo (UNIFESP); Univ TorontoBackground: Misoprostol, a synthetic prostaglandin E1 analog is labeled for the treatment of gastric and duodenal ulcers, in Brazil, where abortion is not a legal procedure, there is a widespread popular misuse of this drug in abortion attempts. This misuse and the fact that, in many cases the desired pregnancy termination does not occur, raise concerns about fetal safety. Case reports of congenital anomalies after maternal use of misoprostol have been published. the objective of this work was to compare pregnancy outcome following misoprostol exposure with a matched control group, This is the first prospective controlled study on fetal safety after misoprostol use. Methods: A prospective, observational cohort study with 86 exposed and 86 pair-matched, non-exposed controls. Results: There was no significant difference in the rates of major or minor birth between exposed compared to non-exposed infants (2/67 vs 2/81, major defects; 7/67 vs. 3/81, minor anomalies) There were significantly more miscarriages in the exposed group (17.1% vs, 5.8%; relative risk, 2.97; 95% confidence interval, 1.12 to 7.88), There was no statistical difference in gestational age at delivery, birth weight, sex ratio, rate of prematurity, low birth weight, or rates of cesarean section between groups. Conclusions: Our study, despite its limited statistical power, does not suggest a potent teratogenic action of misoprostol exposure during pregnancy. (C) 1999 Elsevier Science Inc.
