[UNIFESP]; Magalhaes, Octaviano [UNIFESP]; Maia, Mauricio [UNIFESP]; Humayun, Mark S.; Doheny Eye Inst; Universidade Federal de São Paulo (UNIFESP); Hosp Evangel Curitiba FEMPAR; Univ So CalifPurpose: the purpose of the study was to determine the performance of dual pneumatic ultra high-speed 23-gauge cutters operated with variable duty cycle (DC) settings.Methods: Frame-by-frame analysis of high-speed video was used to determine the DC in core, 50-50, and shave modes. Using three cutters at various cycles per minute and aspiration levels, mass of water or vitreous removed from a vial was measured within a specified time period. Average flow rates were calculated for each aspiration level and cut rate with the different DC options.Results: the DC increased with increasing cut rate in the shave mode was relatively stable in the 50-50 mode and decreased for the core mode. the DC converged at 5,000 cycles per minute for the 3 different modes. Water flow curves followed the DC variation. Vitreous flow rates for all the DC modes increased with increasing cut rates and peaked at 5,000 cycles per minute (P < 0.05). the results of the 50-50 mode, which had isolated the DC influence, showed that increasing aspiration and/or cut rate independently increased the vitreous flow rate.Conclusion: Progressive values of aspiration and/or cut rate increase the vitreous flow rate, independently of the DC. the DC control also has an important effect on the vitreous flow, but this effect was reduced at high cut rates because of convergence of the DC modes. Speech therapy treatment on patients who have a neurogenically induced dysphagia begins at hospital, in the intensive rehabilitation units. This study shows some of the difficulties found dealing with and orienting the family and the patient from a therapy point of view, mainly during his return to home surroundings. Development. To better understand the dysphagic situation, normal swallowing steps (oral pharyngeal and esophageal) steps have been described These were then related to the dysphagic problems, with their evaluation, classification and characteristics. Under dysphagia rehabilitation the therapeutic guidance should be lead by the appropriate nutritional demands and prevent aspirations. Some of the therapeutic procedures for treating and controlling neurogenic dysphagia have been described (head posture control tactile and thermal stimuli, etc.). The author questions the difficulties found controlling orientation and maintaining conduct outside the hospital environment, principally with patients at risk from aspirations.

F.; Iaconelli Júnior, Assumpto [UNIFESP]; Ortiz, V; Borges Júnior, Edson [UNIFESP]; Universidade Federal de São Paulo (UNIFESP); Univ Caxias SulObjective: Our aim was to assess the suppression of spermatogenesis and sperm retrieval rate after testicular sperm extraction (TESE) or testicular sperm aspiration (TESA) in adult rats with surgically induced cryptorchidism. Methods: Adult rats were submitted to TESE and TESA procedures after 15 days of induced cryptorchidism. After spermatozoa retrieval, the testicles were extracted, weighed and a morphological analysis by conventional light microscopy was done. the numbers of spermatozoa retrieved in both TESA and TESE were rated and compared. Results: Histological analysis of the testicles revealed Sertoli cell-only syndrome in 60% of the testicles, and maturation arrest in the remaining cryptorchid testicles. Significant differences were seen in the number of spermatozoa retrieved (P < 0.05) between cryptorchidic and control rats. When sperm retrieval techniques were compared, no differences were detected in the number of spermatozoa obtained (P > 0.05). Conclusions: It seems that a 15 day period of cryptorchidism is enough to induce spermatogenesis disorders. No differences were detected in the number of spermatozoa retrieved in the right or left testicles, irrespective of the testicular pole. Furthermore, and even more importantly, no differences in the retrieval rate were seen between the two techniques.
- ItemSomente MetadadadosSurgery for aspiration: Analysis of laryngotracheal separation in 23 children(Springer, 2006-10-01) Manrique, Dayse; Parenti Settanni, Flavio Aurelio; Brasil, Osíris de Oliveira Camponês do [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)The aim of this study was to analyze the efficacy of laryngotracheal separation (LTS) in eliminating aspiration in children by comparing pre- and postoperative conditions. This prospective study used an internal control group. Children with neurologic impairment and a diagnosis of chronic aspiration were subjected to LTS at the Associacao de Assistencia a Crianca Deficiente (AACD). Twenty-three children had undergone LTS with the modified Lindeman technique. All of them gained complete control of aspiration. Frequency of hospitalization, number of respiratory infections, and level of secretion were statistically reduced. After surgery only 21.7% of the children were capable of oral intake exclusively. LTS is an effective and safe technique that can be used in children resulting in aspiration control in 100% of the patients and without repercussions in the respiratory secretion and pulmonary infections.
