- ItemAcesso aberto (Open Access)Adaptação de prótese auditiva e a privação da audição unilateral: avaliação comportamental e eletrofisiológica(Associação Brasileira de Otorrinolaringologia e Cirurgia Cervicofacial, 2012-12-01) Wieselberg, Margarita Bernal [UNIFESP]; Iorio, Maria Cecilia Martinelli [UNIFESP]; Santa Casa de São Paulo Faculdade de Ciências Médicas; Universidade Federal de São Paulo (UNIFESP)The phenomenon of Late-Onset Unilateral Auditory Deprivation was first reported in 1984. However, a high number of unilateral hearing aid fittings are still carried out in cases of bilateral hearing loss, justified by non-auditory factors such as cost, vanity, misinformation and public health policies. OBJECTIVE: To carry out behavioral and electrophysiological assessment of the auditory performance of adults using unilateral amplification compared with individuals exposed to bilateral symmetric auditory stimulation. METHOD: Thirty five adults, all with symmetric bilateral sensorineural hearing loss, regular users of unilateral hearing aid, bilateral hearing aids and not users of hearing aids, were assessed on behavioral and electrophysiological tests. RESULTS: Variance analysis revealed that in the unilaterally fitted group, P300 latency was significantly greater in ears with auditory deprivation compared with those fitted with the hearing aid (p < 0.05). This same group also had poorer performance on the Sentence Recognition Test in Noise held in free field. CONCLUSION: These results corroborate findings in the literature showing that unilateral auditory deprivation can lead to physiological and perceptual changes.
- ItemSomente MetadadadosBilateral hemifacial spasm: A series of 10 patients with literature review(Elsevier B.V., 2008-01-01) Felicio, Andre Carvalho [UNIFESP]; Godeiro-Junior, Clecio de Oliveira [UNIFESP]; Borges, Vanderci [UNIFESP]; Silva, Sonia Maria de Azevedo [UNIFESP]; Ferraz, Henrique Ballalai [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Bilateral hemifacial spasm (HFS) is a rare movement disorder posing diagnostic doubt with other facial dyskinesias. in this report, we describe clinical, radiological and therapeutic features of 10 patients with bilateral HFS. the prevalence of bilateral HFS in our sample was of 2.6%. Clinical characteristics of our patients did not differ from the classic features of unilateral cases. the mean latency for contralateral facial nerve involvement was of 33.3 months. in only one case a vascular abnormality was seen. We conclude that bilateral HFS is rare and that clinical differentiation with other facial dyskinesias should be promptly made to introduce appropriate therapy. (C) 2007 Elsevier B.V. - ItemAcesso aberto (Open Access)Bilateral SUNCT syndrome associated to chronic maxillary sinus disease(Academia Brasileira de Neurologia - ABNEURO, 2006-06-01) Bichuetti, Denis Bernardi [UNIFESP]; Yamaoka, Wellington Yugo [UNIFESP]; Bastos, João Ricardo Parrela [UNIFESP]; Carvalho, Deusvenir de Souza [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)SUNCT syndrome (short lasting unilateral neuralgiform headache with conjuntival injection and tearing) is defined as short attacks of periorbital unilateral pain and accompanied by ipsilateral lacrimation and redness of the same eye. We present an unusual SUNCT case with bilateral pain that started five years ago after an acute maxillary sinus infection that evolved to chronic sinusitis. This association has been described in few SUNCT cases, but its causal role remains uncertain. The patient was a 58 years old man that fulfilled a headache diary that showed the usual circadian pattern, worsening in the morning and afternoon, and responded to treatment with gabapentina. He was submitted to a functional endoscopic sinus surgery and evolved with milder pain. In a review of 21 patients, 5 had a past medical history of sinusitis, but the causal role of this association remained uncertain.
- ItemSomente MetadadadosLymphangitic sporotrichosis: An uncommon bilateral localization(Kluwer Academic Publ, 1998-01-01) Tomimori-Yamashita, J.; Takahashi, C. H.; Fichman, O.; Costa, E. B.; Michalany, N. S.; Alchorne, MMA; Universidade Federal de São Paulo (UNIFESP)Sporotrichosis is a mycotic disease caused by cutaneous inoculation of the dimorphic fungus Sporothrix schenckii. the primary lesion can spread and often develop a unilateral lymphocutaneous lesions or, rarely, disseminated disease. We report a lymphangitic sporotrichosis case with ulcerated erythematous nodules distributed bilaterally on the posterior and medial aspect of the both legs, probably due to multiple inoculations. the treatment with oral potassium iodide was satisfactory.
- ItemAcesso aberto (Open Access)Úlcera de córnea bilateral por Pseudomonas em usuário de lente de contato descartável(Conselho Brasileiro de Oftalmologia, 1999-12-01) Lipener, César [UNIFESP]; Ribeiro, André Luiz Parolin [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Introduction: Although the use of contact lenses is appa-rently very simple, it is necessary to obey criteria for their correct use, conservation and disposal. Corneal bacterial ulcers are a much-feared complication for users of contact lenses and can be related to various factors such as hygiene, storage and most of all long-term use. Purpose: To describe a case of bilateral ulcer, caused by Pseudomonas in a patient who was a user of disposable lenses. This study highlights and discusses the importance of prior examination, instruction and adequate supervision. In addition it is emphasized that although the lenses are disposable, they are not free from the possibility of serious complications. Case Study: A seventeen-year-old student who had used contact lenses for six months without prior examination or medical prescription complained of pain in the left eye. When he was examined, we observed a corneal ulcer infiltrated in the median periphery. Samples were collected and a treatment with fortified eye-drops was started. After eight hours the patient returned now complaining of pain in the right eye which we examined and observed diffuse keratitis with a mucopurulent secretion. The treatment was the same. Laboratory tests revealed Pseudomonas in both eyes and the ailment was healed within a week's treatment without leaving sequelae. Discussion: The author discuss the factors involved in the occurrence of infectious keratitis in users of contact lenses illustrating that even though disposable lenses are appa-rently very simple they can cause serious problems. It also demonstrates that the long-term use of contact lenses is considered to be a main risk factor. The description of a case of bilateral keratitis is a warning about the use of this kind of lens without proper orientation and supervision.
