OBJECTIVE: Describe melanocytic lesions (number and anatomical distribution) in skin types V and VI (ST V/VI) compared to skin types I and II (ST I/II) according to Fitzpatrick's classification. Identify differences in dermoscopic findings of acquired melanocytic nevi (global pattern, pigment and color distribution) between the groups. METHODS: Cross-sectional, prospective and consecutive data collection in two dermatological outpatient clinic, between October 8, 2010, and March 20, 2013. From the 501 volunteers, 480 participants fullfilled the eligibility criteria. A total of 460 acquired melanocytic nevi were selected for dermoscopy analysis. RESULTS: The individuals with ST V/VI had less melanocytic lesions than those with ST I/II (15,08 vs 7,90, p=0,032), and the anatomical distribution in the first group was predominantly in the face and acral sites (p<0,001). The AMN in the group ST V/VI were associated with reticular pattern (p<0.0001), tendency to central hyperpigmentation (p=0.0025). LIMITATIONS: Choice of a single representative nevus per patient. CONCLUSION: The AMN in the individuals with ST V/VI has a distinct dermoscopic pattern from those with ST I/II.

[UNIFESP]; Yamada, Sergio [UNIFESP]; Pellacani, Giovanni; Rezze, Gisele G.; Universidade Federal de São Paulo (UNIFESP); AC Camargo Canc Ctr; Univ Modena & Reggio EmiliaBackground: Pigmented actinic keratosis (PAK) is a frequent simulator of lentigo maligna (LM) on the face upon clinical and dermoscopic examination, leading to misdiagnosis and unnecessary excisions. LM and PAK share dermoscopic features, making it difficult to have a confident diagnosis of PAK only with current dermoscopic knowledge.Objective: We sought to evaluate sensitivity, specificity, and interobserver reproducibility of a novel dermoscopic feature, inner gray halo (IGH), and establish its histopathological and confocal correlations.Methods: Dermoscopists blinded to histopathological diagnosis evaluated 58 PAK and 21 LM for the presence of IGH and dermoscopy parameters. Areas exhibiting IGH were marked and imaged with reflectance confocal microscopy before sampling for histopathologic correlation. Reflectance confocal microscopy and transverse histologic sectioning were performed in 14 of 79 cases.Results: IGH was present in 53 of 58 (94.1%) PAK and in 5 of 21 (23.8%) LM in our series (sensitivity 91.4%; specificity 71.4%; positive predictive value 89.8%). Interobserver agreement was excellent (Kappa 0.846). Through transverse and perpendicular histologic sections, a dermoscopic-histologic-confocal correlation of IGH was established.Limitations: A larger test set is needed to further validate the use of IGH in the differential diagnosis of PAK and facial pigmented lesions.Conclusion: IGH is a novel dermoscopic parameter useful for the differentiation of PAK from LM on the face.

Methods: 482 Fitzpatrick skin types IV, V or VI patients, between 19 and 72 years old, were clinically evaluated and underwent nail bed dermoscopic examination. 58 patients who showed longitudinal melanonychias and dermoscopic patterns with brown or black coloration of the background were submitted to nail matrix biopsy. When found in more than one digit, biopsy was performed in the darkest coloration lesion presented in dermoscopy. Results: All skin types IV, V or VI patients who had longitudinal melanonychias, which show brown or black coloration of the background in the dermoscopic examination, releaved histopathological analysis compatible with melanocytic activation. Conclusions: Racial melanonychias in skin types IV, V or VI patients, resulting from melanocytic activation, may present brown or black coloration of the background in the dermoscopic examination.
- ItemAcesso aberto (Open Access)Shiny White Streaks: A Sign of Malignancy at Dermoscopy of Pigmented Skin Lesions(Acta Dermato-venereologica, 2014-01-01) Shitara, Danielle [UNIFESP]; Ishioka, Priscila [UNIFESP]; Alonso-Pinedo, Yarel; Palacios-Bejarano, Leyla; Carrera, Cristina; Malvehy, Josep; Puig, Susana; Hosp Clin Barcelona; Universidade Federal de São Paulo (UNIFESP); Inst Dermatol Jalisco Dr Jose Barba Rubio; Inst Salud Carlos IIIThe aim of this study was to evaluate the practical importance of the presence of shiny white streaks (SWS) (chrysalis or crystalline structures in polarized dermoscopy) for suspicion of malignancy, diagnosis of melanoma, and pre-operative estimation of Breslow thickness and its correlation with total dermoscopy score (TDS). SWS were present in 13.6% of 800 consecutive excised lesions. the presence of SWS was associated with malignancy (odds ratio (OR) 10.534, 95% confidence interval (95% Cl) 6.357-17.455, p<0.0005), in the context of melanocytic lesions with invasive melanoma (OR 10.333, 95% CI 3.812-28.014) and melanomas with high TDS (OR 6.286, 95% CI 1.673-23.619), but was also a factor in the diagnosis of featureless and some thin melanomas. These results corroborate the clinical applicability of SWS in aiding the diagnosis of malignancy and helping to raise the general dermatologist's awareness in cases of doubt and featureless lesions.
- ItemAcesso aberto (Open Access)Spitz nevus and Reed nevus(Sociedade Brasileira de Dermatologia, 2012-06-01) Yoradjian, Alessandra; Enokihara, Mílvia Maria Simões e Silva [UNIFESP]; Paschoal, Francisco Macedo [UNIFESP]; ABC School of Medicine; Universidade Federal de São Paulo (UNIFESP)Spitz and Reed nevi, also called spindle and/or epithelioid cell nevi, are a special group of melanocytic lesions due to their peculiar clinical, dermoscopic and histopathological features. The study of these nevi is of great interest because both their nomenclature and classification are still a matter of discussion. Most importantly, the difficulty to differentiate them from melanoma can lead to inadequate therapies. In this context, dermoscopy, a link between clinical and anatomopathological examinations, appears as a helpful diagnostic tool whose accuracy can reach 93%. Borderline lesions are still a great challenge and object of research, including molecular studies. The present study explores the relevant characteristics of these nevi, with emphasis on dermoscopic findings, aiming at understanding their natural history, as well as discussing treatment and patient follow-up.
