- ItemSomente MetadadadosCausa clínicas de eletrorretinograma negativo em pacientes de hospital universitário(Universidade Federal de São Paulo (UNIFESP), 2015-12-31) Rocha, Daniel Martins [UNIFESP]; Berezovsky, Adriana [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Objective: To determine the frequency and cause of electroretinography (ERG) negative scotopic over a period of 11 years and correlate the fundus findings, age, sex, visual acuity and family inbreeding. Methods: This retrospective cross-sectional observational study included 1612 patients, who were divided into two groups according to age, with 245 of the group of children and adolescents under the age of eighteen years of age (group 1) and in 1367 the adult group over the age of eighteen years of age (group 2) examined between March 2004 and July 2015 in Visual Electrophysiology Laboratory of the Department of Ophthalmology and Visual Sciences at the Federal University of São Paulo / Hospital Sao Paulo. Patients were selected who had negative ERG, or a significant reduction of amplitude of the b-wave so that the wave amplitude was lower than it, i.e., b / a ratio ? 1 at maximum scotopic ERG response. The level of statistical significance was taken as p ? 0.05. Results: Of the total sample, 41 (2.5%) patients had negative ERG, 27 adults (2.0%) and 14 children and adolescents (5.7%). Ages ranged from 0.5 to 78.1 years (mean 32.8 ± 20.5; median 28.6), and for group 1 0.5 to 17.0 years (12.2 ± 4.6 ; median 13.8) and for Group 2, 18.6 to 78.1 years (43.5 ± 17.0, median 38.5). Of the total, 23 patients (56.1%) were male and 18 (44.9%) were female. ERG negative bilateral was found in 29 patients (12 in Group 1, 17 in Group 2) and unilateral in 12 patients (Group 1 2, 10 for group 2). A family history of consanguinity was found in nine patients (21.9%). The average visual acuity in the better eye sight was 0.54 ± 0.54 logMAR of (20/70) and 0.73 ± 0.39 logMAR (20/107) for group 1 and 0.45 ± 0, 58 logMAR (20/56) for group 2. The visual acuity for the worst eye vision was 0.88 ± 0.69 logMAR (20/150) and 0.83 ± 0.41 logMAR (20 / 135) for group 1 and 0.91 ± 0.74 logMAR (20/186) for group 2. The changes most commonly found in the fundus were: atrophy of the retinal pigment epithelium in 34 patients (12 in group 1; 22 Group 2), pale papilla in 22 patients (14 in group 1, group 2, 8) and vascular attenuation of 22 patients (8 in group 1: 14 group 2). The most frequent diagnostic hypotheses were: retinitis pigmentosa (33.3%), juvenile retinoschisis (12.8%) and uveitis (12.8%). Conclusions: The negative ERG frequency in the 11-year study period was close (3-5%) described in the literature, is more common in males and in children and adolescents. Visual acuity was worse for children and adolescents than adults. Bilateral cases were more common than unilateral. The retinitis pigmentosa and juvenile retinoschisis were the most frequent diagnostic hypotheses.
