- ItemAcesso aberto (Open Access)O ensino da comunicação na graduação em medicina: uma abordagem(UNESP, 2006-06-01) Rossi, Pedro Santo; Batista, Nildo Alves [UNIFESP]; Secretaria de Saúde, Estado de São Paulo Departamento Psiquiátrico II; Universidade Federal de São Paulo (UNIFESP)The purpose of this work is to analyze the process of teaching/learning communication within the doctor-patient relationship during the undergraduate medical course, discussing the ideas of students and of coordinators regarding this process, and identifying how and when the curriculum takes this issue into account. Twelve graduates and nine course coordinators were interviewed. We learned that there is a great diversity of ideas on communication, especially a tendency to think of it as an instrumental skill for obtaining information and making oneself understood within medical procedures. The learning of communication takes place primarily in an implicit way within the education of physicians, being connected with certain disciplines, such as Semiology and Medical Psychology, or being imbued in the curriculum during the different moments of teaching and learning. The observation of attitudes and behaviors in the daily practice of teaching, whether by professors or other physicians in practice, notably during internship, is the main factor responsible for student learning of this skill. The results found in this research, given the relevance of communication in the exercise of medical practice, indicate that it is necessary to reassess this theme in connection with the pedagogical practices used to train future physicians.

Methods: This was across-sectional analytical study. The term ?caesarean delivery? and 25 synonyms were entered into the 5 most popular search engines in Brazil (google.com.br; bing.br.com; google.com; br.ask.com e br.search.yahoo.com) using the browser Google Chrome. The first 3 pages of hits were downloaded and assessed for possible inclusion. Freely available Web pages written in Portuguese in a language accessible for lay persons and which presented at least two paragraphs with information on CS were analyzed by two independent investigators using the Discern instrument to assess quality and a content checklist to assess completeness of information on CS. Results: 3900 Webpages were retrieved and 176 fulfilled the selection criteria. The overall average Discern score was 43.6 (+ 8.9 SD), of a maximum score of 75. Overall, 30% of the pages were of very poor or poor quality (total score < 39) and 47% were of regular quality (total score 39-50). Most pages scored low especially in questions related to reliability of the information presented. The most frequently covered topics were indications for CS (80% of Páginas Web), which did not reflect clinical practice, short-term maternal risks (80%) and potential benefits of CS (56%), including maternal and doctor convenience. Less than half of the Páginas Web mentioned perinatal risks and less than one third mentioned longterm maternal risks associated with CS such as uterine rupture (17%) or placenta praevia/accreta (12%) in future pregnancies. Conclusions: The quality and completeness of Web-based resources in Portuguese about CS were poor to regular. Pending improvement of these resources, Brazilian obstetricians should warn their patients about these facts and encourage them to discuss what they have read on the net about CS.
- ItemSomente MetadadadosTradução e validação do questionário de empatia warmometer para o português brasileiro(Universidade Federal de São Paulo (UNIFESP), 2016-07-31) Carvalho, Marieta Sodre de Mello [UNIFESP]; Nakamura, Mary Uchiyama Nakamura [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Introduction: The doctor-patient relationship (PPR) is now one of the key points to the improvement of health care, as it helps to improve diagnostic accuracy, cooperation and collaboration of the patient with the proposed treatment, lesser rates of depression, improvement of quality of life and a more active participation of the patient in prevention and health promotion. Among others, clinical empathy plays a key role of the PPR and it showcases the physician?s ability to understand the environment in which the patient lives, including their perspective in life and emotions. Objective: Translation and validation of the clinical empathy questionnaire "WARMOMETER" to the Brazilian Portuguese. Methods: This study was conducted over six steps, which are: 1) Author?s approval, 2) Ethics Committee approval, 3) Translation into Portuguese according to the protocol suggested by the author of the original article 4) Cultural equivalence, 5) Assessment of measurement properties and 6) Statistical analysis. Results: 32 pregnant patients from the UNIFESP Pre-natal Anthroposophy Clinic participated in the study. The measurement properties showed good agreement for the inter-observers, while the evaluation for intra-observers showed poor correlation. There was a poor correlation between the self-assessment of physician empathy (IRI) and the assessment made by the patient (WARMOMETER). When compared with the gold standard questionnaire, CARE, p < 0.001 was observed. Conclusion: The WARMOMETER questionnaire translated into Brazilian Portuguese is a valid and sensitive instrument for empathy medical evaluation in second person.
