- ItemSomente MetadadadosAnticoagulation for atrial fibrillation: Underutilization in a Brazilian tertiary outpatient clinic(Clinical Cardiology Publ Co, 2004-11-01) Mesas, Cezar Eumann [UNIFESP]; Veloso, Henrique Horta [UNIFESP]; De Paola, Angelo Amato Vincenzo [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Despite a clear benefit of anticoagulation in patients with atrial fibrillation for the prevention of stroke, treatment rates are disappointingly low in clinical practice in the industrialized countries. This survey demonstrates similar rates in a Brazilian tertiary outpatient clinic, with only 55% of patients at high risk receiving dose-adjusted warfarin. Poor patient education and healthcare system limitations are considered important barriers. Strategies to improve treatment rates should target specific problems at each location.
- ItemSomente MetadadadosEffects of Compounds from Passiflora edulis Sims f. flavicarpa Juice on Blood Coagulation and on Proteolytic Enzymes(Bentham Science Publ Ltd, 2012-05-01) Sato, Ana Claudia [UNIFESP]; Andrade, Sonia Aparecida de; Brito, Marlon Vilela de [UNIFESP]; Miranda, Antonio [UNIFESP]; Sampaio, Misako Uemura [UNIFESP]; Maffei, Francisco Humberto de Abreu; Oliva, Maria Luiza Vilela [UNIFESP]; Universidade Federal de São Paulo (UNIFESP); Inst Butantan; Universidade de São Paulo (USP); Hosp Santa CatarinaPassion fruit (Passiflora edulis Sims f. flavicarpa) is popularly known for its sedative and calming properties and is consumed as a fresh fruit or as a juice. The clinical observation of blood incoagulability associated with excessive consumption of passion fruit juice, in a patient treated with warfarin, prompted the current study to investigate in vitro the presence of blood clotting inhibitors in Passiflora edulis Sims f. flavicarpa extract. After purification process, two compounds of distinct molecular weight and inhibitory action were better characterized. One is a trypsin inhibitor similar to inhibitors from Bowman-Birk family, named PeTI-I12, and other is a compound active in coagulation that prolongs aPTT and PT, but does not change TT. The aim of this study is to provide evidence that passion fruit extract's components play a role on hemostasis and therefore may be relevant in the handling of patients treated with anticoagulants or suffering hemorrhagic diseases.
- ItemSomente MetadadadosGenotype-guided warfarin therapy: current status(Future Medicine Ltd, 2018) Tavares, Leticia C.; Marcatto, Leiliane R.; Santos, Paulo C. J. L. [UNIFESP]Warfarin pharmacogenomics has been an extensively studied field in the last decades as it is focused on personalized therapy to overcome the wide interpatient warfarin response variability and decrease the risk of side effects. In this expert review, besides briefly summarizing the current knowledge about warfarin pharmacogenetics, we also present an overview of recent studies that aimed to assess the efficacy, safety and economic issues related to genotype-based dosing algorithms used to guide warfarin therapy, including randomized and controlled clinical trials, meta-analyses and cost-effectiveness studies. To date, the findings still present disparities, mostly because of standard limitations. Thus, further studies should be encouraged to try to demonstrate the benefits of the application of warfarin pharmacogenomic dosing algorithms in clinical practice.
