Introduction: the experience of accidents with biological material generates specific concern and care demands, however, when it involves health professionals who care for people living with HIV/AIDS (PLWHA), there is a need to consider even more complex repercussions. Objective: the research sought to understand the repercussions of work accidents with biological material of health professionals of a specialized service in HIV/AIDS, considering personal, family, social aspects and impacts on work capacity. Method: a qualitative research was conducted at the Infectious Diseases Control Coordination (CCDI) of the Municipal Health Department of Santos - SP, in which there was a screening of this type of accident with health professionals who take care of PLWHA in this coordination currently, considering the occurrence of the accident in the period from 2000 to 2019. Thus, the principal researcher conducted an in-depth individual interview and construction of the life history technique of seven health professionals identified in this context. The interviews were categorized by the thematic Content Analysis technique. A documentary research was carried out in the medical records of work accidents of the service to support the composition of the contexts and outcomes of the accident. Results: seven health professionals who currently work at the coordination specialized in HIV/AIDS in Santos-SP and who suffered an accident with biological material between 2000 and 2019 were identified and interviewed. Three general categories of thematic analysis were constructed: conceptions about health; experienced professional situations and aspects related to work accidents. In turn, the participants' life stories revealed that the experience with the accident was remarkable and that personal aspects, such as fears and expectations, interfered in the way each one reacted initially and later to what happened. The speeches were observed in the gaps of knowledge and risk behaviors in relation to the issue of accidents with biological material, as well as references to insufficiency in welcoming, especially emotional, and the absence of participation in training on the theme offered in the service. The PCDT-PEP does not provide direct indications about the importance of emotional support/support in these cases. Final considerations: it is expected that this research will offer subsidies to rethink protocols and care processes of health professionals who work in specialized services in HIV/AIDS and who have suffered accidents with biological material, considering not only the biomedical aspects of exposure, but also the support to subjective aspects, aiming at the integral health care of these workers. In this sense, we seek the construction of a truly fruitful care, a care that envisions the notion of protecting the dignity of the one who cares, thus achieving their happiness and realization of their capacities in a healthy way.
