Adaptação transcultural e validação para o Português do Brasil da escala “Objective Structured Assessment of Laparoscopic Salpingectomy” (OSA-LS)

O ensino cirúrgico estrutura-se no aprendizado com educação, treinamento e avaliação baseados em evidência e proficiência. Nesse contexto, a avaliação de cirurgiões ginecológicos em laparoscopia é um desafio. A escala OSA-LS (Objective Structured Assessment of Laparoscopic Salpingectomy) foi criada em 2008 com a proposta de avaliar o desempenho em cirurgias laparoscópicas ginecológicas. Trata-se de um método sistemático, estruturado e objetivo para avaliar o cirurgião em treinamento e assim, para ter esta ferramenta disponível para uso no Brasil propusemos a adaptação transcultural. O objetivo deste trabalho foi traduzir, adaptar e validar para a língua portuguesa do Brasil a escala OSA-LS. Seguimos a metodologia proposta pela Organização Mundial da Saúde e após concluir as etapas de tradução, reunião com especialistas, retrotradução e ajustes finais, chegamos a uma escala disponível para ser validada em nossa realidade. Na etapa de validação, foram utilizados vídeos de salpingectomias laparoscópicas realizadas por cirurgiões iniciantes, intermediários e experientes. Os vídeos foram coletados entre agosto/2018 e março/2020, fornecidos por cirurgiões que operaram pacientes acompanhadas no ambulatório de Videolaparoscopia Ginecológica do Hospital São Paulo – UNIFESP. Três laparoscopistas ginecológicos experientes utilizaram a escala OSA-LS traduzida na avaliação dos vídeos e opinaram sobre o entendimento da mesma. Os avaliadores não encontraram dificuldades ao utilizar a versão adaptada transculturalmente da escala OSA-LS. A avaliação independente e cega dos vídeos demonstrou que a ferramenta foi capaz de diferenciar bem os grupos, com notas bastante distintas entre iniciantes, intermediários e experientes. Entretanto, a medida do grau de concordância entre os três observadores mostrou uma variação importante. Este achado limita o uso da escala em nosso meio como uma ferramenta objetiva capaz de avaliar o aprendizado laparoscópico. Apesar do processo de tradução e adaptação transcultural da escala OSA-LS para o português do Brasil ter sido finalizado com sucesso, o uso desta escala em nossa realidade não se mostrou válido, com grande discrepância entre os avaliadores.
Surgical teaching is structured on learning with education, training and assessment based on evidence and proficiency. In this context, the evaluation of gynecological surgeons in laparoscopy is a challenge. The OSA-LS scale (Objective Structured Assessment of Laparoscopic Salpingectomy) was created in 2008 with the purpose of evaluating performance in gynecological laparoscopic surgeries. It is a systematic, structured and objective method to evaluate the surgeon in training and to have this instrument available for use in Brazil, we proposed a cross-cultural adaptation. The objective of this work was to translate, adapt and validate the OSA-LS scale into Brazilian Portuguese. We followed the methodology proposed by the World Health Organization and after completing the translation steps, meeting with specialists, back-translation and final adjustments, we arrived at a scale available to be validated in our reality. In the validation stage, videos of laparoscopic salpingectomies performed by beginning, intermediate and experienced surgeons were used. The videos were collected between August/2018 and March/2020, provided by surgeons who operated on patients monitored at the Gynecological Videolaparoscopy outpatient clinic of Hospital São Paulo - UNIFESP. Three experienced gynecological laparoscopists used the translated OSA-LS instrument to evaluate the videos and gave their opinion on the understanding of the scale. The evaluators did not find difficulties when using the cross-culturally adapted version of the OSA-LS instrument. The independent and blind evaluation of the videos demonstrated that the instrument was able to differentiate the groups well, with very different scores between beginners, intermediates and experts. However, the measurement of the degree of agreement between the three observers showed an important variation. This finding limits the use of the scale in our setting as an objective tool capable of assessing laparoscopic learning. Despite the successful completion of the process of translation and cross-cultural adaptation of the OSA-LS scale into Brazilian Portuguese, the use of this scale in our reality did not prove to be valid, with great discrepancy between evaluators
