- ItemAcesso aberto (Open Access)Fatores associados à recidiva em hanseníase em Mato Grosso(Faculdade de Saúde Pública da Universidade de São Paulo, 2011-08-01) Ferreira, Silvana Margarida Benevides [UNIFESP]; Ignotti, Eliane; Gamba, Mônica Antar [UNIFESP]; Universidade de Cuiabá Faculdade de Enfermagem; Secretaria Municipal de Saúde de Cuiabá; Universidade Estadual de Mato Grosso; Universidade Federal de São Paulo (UNIFESP)OBJECTIVE: To analyze factors associated with relapse of leprosy. METHODS: Retrospective case-control study including 159 patients older than 15 diagnosed with leprosy attending reference centers for leprosy in five municipalities in the state of Mato Grosso, central-western Brazil. Cases (n=53) were patients with relapsed leprosy diagnosed from 2005 to 2007 who were compared with controls (n=106) matching for gender and operational classification who were considered cured after treatment in 2005. Data was obtained from the local Notifiable Diseases Database, medical records and interviews. For the analyses conditional logistic regression and hierarchical approaches were used. RESULTS: After adjustment, the following factors were associated with relapse of leprosy: living in rental housing (OR = 4.1; 95%CI: 1.43;12.04); living in houses constructed of wood and mud (OR = 3.2; 95%CI: 1.16;8.76); living with dwellings with more than five people (OR = 2.1; 95%CI: 1.03;4.36); alcohol use disorder (OR = 2.8; 95%CI: 1.17;6.79); irregular treatment (OR =3.8; 95%CI: 1.44;10.02); lack of knowledge about the disease/treatment (OR = 2.6; 95%CI: 1.09;6.13); use of public transportation to get to the clinic (OR = 5.5; 95%CI: 2.36;12.63); clinical form of the disease (OR = 7.1; 95%CI: 2.48;20.52), and treatment regimen (OR = 3.7; 95%CI: 1.49;9.11). CONCLUSIONS: The predictive factors of relapse are associated with housing conditions, living habits, organization of health services, clinical forms of leprosy and treatment regimen. Health services should educate patients on the disease as well as ensure consistent treatment.
- ItemAcesso aberto (Open Access)Fatores dietéticos e câncer oral: estudo caso-controle na Região Metropolitana de São Paulo, Brasil(Escola Nacional de Saúde Pública Sergio Arouca, Fundação Oswaldo Cruz, 2007-03-01) Marchioni, Dirce Maria Lobo; Fisberg, Regina Mara; Góis Filho, José Francisco de; Kowalski, Luiz Paulo [UNIFESP]; Carvalho, Marcos Brasilino de; Abrahão, Márcio [UNIFESP]; Latorre, Maria do Rosario Dias de Oliveira; Eluf Neto, José; Wünsch-Filho, Victor; Universidade de São Paulo (USP); Instituto do Câncer Arnaldo Vieira de Carvalho Departamento de Cabeça e Pescoço; Hospital Heliópolis Departamento de Cabeça e Pescoço; Universidade Federal de São Paulo (UNIFESP)Dietary factors associated with oral cancer were investigated in a case-control hospital-based study in the city of São Paulo, Brazil, from 1998 to 2002. A total of 835 subjects, 366 with histologically confirmed incident cases of oral-cavity or pharyngeal cancer and 469 controls participated in the study. Dietary data were collected with a food frequency questionnaire (FFQ). Odds ratios (OR) and 95% confidence intervals (95%CI) were obtained from unconditional logistic regression, adjusted for potential confounders. Inverse associations with risk of oral cancer were found for the highest intake of beans, OR = 0.37 (95%CI: 0.22-0.64), raw vegetables, OR = 0.51 (95%CI: 0.29-0.93) and the intermediate tertile of rice and pasta intake, OR = 0.56 (95%CI: 0.38-0.83). Positive associations were observed for the highest intake of eggs, OR = 1.84 (95%CI: 1.23-2.75), potatoes, OR = 2.22 (95%CI: 1.53-3.25), and milk, OR= 1.80 (95%CI: 1.09-2.98). Some traditional Brazilian foods like rice and beans emerged as protective factors against oral cancer, independently of other risk factors.
- ItemAcesso aberto (Open Access)Gravidez na adolescência e uso freqüente de álcool e drogas no contexto familiar(Faculdade de Saúde Pública da Universidade de São Paulo, 2008-06-01) Caputo, Valeria Garcia [UNIFESP]; Bordin, Isabel Altenfelder Santos [UNIFESP]; Faculdade de Medicina de Marília; Universidade Federal de São Paulo (UNIFESP)OBJECTIVE: To assess individual and family factors associated to teenage pregnancy, including frequent use of alcohol and illicit drugs by family members. METHODS: Case-control study conducted with a sample of 408 sexually active female adolescents (aged 13-17 years) in school from the city of Marília (Southeastern Brazil) in 2003-2004. Cases consisted of 100 primigravid teenagers assisted in prenatal care programs in health units. Controls were 308 nulligravid students from state public schools. Standardized instruments identified demographic and educational factors, contraceptive behavior, mental health problems, and family characteristics. Statistical analysis included chi-square tests and logistic regression models. RESULTS: Low paternal education (p=0.01), lack of information on sexuality and fertilization (p=0.001) and the use of illicit drugs by a resident family member (p=0.006) were independent risk factors. Family income per capita and asking the partner to use a condom were confounders. CONCLUSIONS: The frequent use of illicit drugs by a resident family member is a factor strongly associated to teenage pregnancy, regardless of other risk factors. The expectation of going to college constitutes a protective factor, mainly in the presence of low maternal education.
- ItemAcesso aberto (Open Access)Oral health, hygiene practices and oral cancer(Faculdade de Saúde Pública da Universidade de São Paulo, 2008-06-01) Marques, Luzia A; Eluf-Neto, José; Figueiredo, Rejane A O; Góis-Filho, José F de; Kowalski, Luiz Paulo [UNIFESP]; Carvalho, Marcos B de; Abrahão, Márcio [UNIFESP]; Wünsch-Filho, Victor; Universidade de São Paulo (USP); Instituto do Câncer Arnaldo Vieira de Carvalho Departamento de Cabeça e Pescoço; Hospital do Câncer A.C.Camargo Departamento de Cabeça, Pescoço e Otorrinolaringologia; Hospital Heliópolis Departamento de Cabeça e Pescoço; Universidade Federal de São Paulo (UNIFESP)OBJECTIVE: To assess the association between oral health and hygiene practices and oral cancer. METHODS: Hospital-based case-control study in the metropolitan area of São Paulo, southeastern Brazil, from 1998 to 2002. A total 309 patients with squamous cell carcinoma of the mouth and the pharynx and 468 controls matched by sex and age were included in the study. Cases were recruited in seven reference hospitals and controls were selected in five out of the seven participating hospitals. Detailed information on smoking, alcohol consumption, schooling, oral health status and hygiene practices were obtained through interviews. Odds ratios (OR) and 95% confidence intervals (95% CI), adjusted by sex, age, schooling, smoking, alcohol consumption as well as the variables oral health status and hygiene practices were estimated using unconditional logistic regression analyses. RESULTS: The use of complete dental prosthesis was not associated with oral cancer but regular gum bleeding showed a strong association (OR 3.1; 95% CI 1.2-7.9). Those who never attended a dental visit were more likely to have oral cancer (OR 2.5; 95% CI 1.3-4.8). Daily mouthwash use showed a stronger association to pharynx (OR 4.7; 95% CI 1.8-12.5) than mouth cancer (OR 3.2; 95% CI 1.6-6.3). CONCLUSIONS: Gum bleeding, no dental care, and daily mouthwash use were factors associated with oral cancer regardless of tobacco and alcohol consumption.
