Forty caregivers of elderly individuals in a home care program in the city of São Paulo (Brazil) were interviewed. QOL was measured using the Brazilian version of the Medical Outcomes Study 36-Item Short-Form Health Survey (SF-36). Socio-demographic characteristics and variables related to the support network offered to the caregiver and to the act of providing care were obtained through a complementary questionnaire. Linear regression analysis demonstrated an independent relation between greater number of hours dedicated to care and three SF-36 subscales: functional capacity, physical aspects and emotional aspects. Having more than eight years of schooling denoted a better score on the general health state subscale and a worse score on the social aspects subscale. Caregivers over 60 years of age achieved a worse score on the physical aspects subscale and women achieved a worse score on the pain subscale. Sons/daughters or spouses who take care of their parents or partners achieved a worse score on the emotional aspects domain. Caregivers who had changed their routine in order to offer care achieved a worse score on the mental health subscale. Socio-demographic factors, the support network offered to the caregiver, and factors related to the dynamics of care can have a negative influence over the QOL of the main caregivers of elderly individuals in home care.

Nesta modalidade, os cuidados geralmente são fornecidos por familiares que muitas vezes não possuem nenhuma capacitação formal. Por isso, a preocupação com os desafios enfrentados pelos cuidadores é fundamental para que a assistência domiciliar a saúde possa ser adequada e atinja o seu propósito. Teve-se como objetivo deste estudo compreender as principais dificuldades relatadas pelos cuidadores familiares dos pacientes em acompanhamento em um serviço de AD. A pesquisa de natureza exploratória e descritiva, com abordagem qualitativa foi realizada com 04 cuidadores familiares em acompanhamento pelo Programa Melhor em Casa no município de Santos/SP. Os dados foram produzidos por meio de um roteiro de entrevista semiestruturado contendo questões relevantes sobre capacitação e realização dos cuidados. Após transcritas e analisadas, as entrevistas foram classificadas por núcleos temáticos. A repetição e o agrupamento dos dados geraram os seguintes temas para análise: A falta de conhecimento; medo, incerteza e a sobrecarga; e a necessidade de capacitação adequada. As falas revelaram algumas das dificuldades encontradas pelos cuidadores que em muitas situações não acreditaram estar preparados para desempenhar tais atividades relacionadas ao cuidado. Além disso, foi um consenso entre os participantes da pesquisa a necessidade da construção de programas de capacitação para os cuidadores domiciliares. A capacitação insuficiente e a falta de conhecimento específico podem interferir na qualidade de vida do paciente e causar consequências a saúde cuidador. Uma proposta para minimizar os impactos seria a implementação de um programa de capacitação com encontros periódicos com os profissionais de saúde. Esses encontros poderiam proporcionar esclarecimento de dúvidas quanto ao manejo e o cuidado com o paciente.

The objective of this research is to analyze the perception of caregivers and family members about nutritional therapy in palliative care of patients in home care. This is a study with qualitative, descriptive, exploratory approach in understandable minute, with family members and caregivers of patients attended by the program Best at Home - APS Santa Marcelina, Basic Health Units, located in the Itaquera, in São Paulo city. Three conversation wheels and 12 individual semi-structured interviews were conducted, using a guiding script for both data collection processes, which were recorded, transcribed and systematized using the thematic content analysis technique. This project was submitted for evaluation by the Ethics and Research Committee of the Federal University of São Paulo, and as co-participant the Municipal Secretary of Health of São Paulo - SP. Exclusion criteria included return by the caregiver. Free and Informed Consent Form without the signature. The profile of the caregivers participating in the research (25), are female (76%), aged between 48 and 58 years (44%), have completed high school (52%), are family members of the patient (96% ). The interpretation of the thematic nuclei of meaning, in which emerged the categories Caring in palliative care, The new condition of the patient, Nutritional treatment and Protagonism of the caregiver. These researches results demonstrated by the analysis of the categories will contribute to the elaboration of a technical production, through the consensus in the answers of the caregivers. This consensus was not in the expectation of the researcher, who hypothetically hypothesized that caregivers could present different perceptions about this theme. This fact allowed the researcher to become a listener and welcome all the reports of the social actors of the research, which contributed to the technical production of this study, which demonstrates the importance of the process of health education, individual and collective, in order to provide autonomy, empowerment and the protagonist of the individual in the construction of the Therapeutic Project, not only for the patient, but also for the caregiver and family, and these as an integral part of AD. The technical production of this study was the creation of a hybrid method for the pedagogical process in Health Education, a video dedicated to caregivers to guide common complications, and the elaboration of a report with the suggestions of caregivers to be delivered to the management of the APS Santa Marcelina Best Home Program in the Itaquera, in São Paulo-SP.
