No entanto, o pós-operatório imediato de lipoaspiração é doloroso e requer analgesia. Estimulação elétrica nervosa transcutânea (TENS) é uma terapia adjunta e analgésica efetiva na dor pós-operatória de cirurgias abdominais, torácicas e ginecológicas. Todavia, de acordo com a literatura, não foram encontrados estudos que avaliaram a efetividade da TENS para dor pós-operatória de lipoaspiração. Objetivo: Avaliar a efetividade da TENS como terapia adjunta na redução da intensidade da dor pós-operatória de lipoaspiração. Métodos: Este estudo é um ensaio clínico randomizado e placebo-controlado, duplo cego, comparando analgesia com TENS ou estimulação placebo em 42 pacientes no pós-operatório de lipoaspiração que foram alocados aleatoriamente em dois grupos de intervenção. Todos os sujeitos receberam 0,1 mg/kg de morfina e 1g de dipirona imediatamente após a cirurgia. A estimulação foi aplicada uma vez, duas horas após a lipoaspiração. O desfecho primário foi: intensidade da dor, redução de pelo menos dois pontos na escala visual analógica (EVA) após a intervenção. Os desfechos secundários foram: necessidade de analgésicos durante as primeiras 24 horas, número de retiradas devido a efeitos adversos da TENS e tipos de efeitos adversos, qualidade da dor, sucesso e satisfação com o tratamento. Resultados: Quarenta e dois sujeitos completaram o estudo e oito sujeitos foram excluídos antes da randomização. Ambos os grupos tiveram redução de pelo menos dois pontos na EVA. Houve diferença significativa (p<0,001, tamanho do efeito 0,92) no grupo TENS ativo, quando comparado ao grupo TENS placebo. O grupo TENS placebo fez maior uso de analgésicos (p<0,001). Não houve retiradas e não foram observados efeitos adversos no grupo TENS ativo, 33,3% do grupo TENS placebo relataram sonolência e náusea. Qualidade de dor, analisada pelo questionário McGill versão brasileira, mostraram diferenças significantes (p<0,001) no grupo TENS ativo obtendo sucesso do tratamento e satisfação após intervenção. Conclusão: TENS como terapia adjunta é efetiva na redução da dor pós-operatória de lipoaspiração

According to World Health Organization statistics, 39% of the population is overweight, and these rates are even higher in Brazil. Like any surgical procedure, this is not risk free, and patient safety is a major concern in the face of the growing demand for plastic surgeries and the high rates of overweight in the population. Objective: To carry out a systematic review of the impact of overweight and obesity on complications in liposuction surgeries. Methods: Electronic searches were performed at MEDLINE, LILACS and CENTRAL; there being no restriction of language, date and gender. Randomized, almost randomized clinical trials, as well as prospective and retrospective cohort studies were selected. Results: Applied to the database search strategy, 431 studies were identified, of which 393 were MEDLINE, 15 were LILACS and 23 CENTRAL. Twenty-one duplicate studies were excluded, leaving 410 studies. Twenty-three were selected for textual analysis, remaining only one. Conclusion: The data found are insufficient to affirm that overweight and obesity impact in complications in liposuction surgeries, it is necessary to carry out new clinical studies, in order to clarify the questioning.

Objective: Introduce an innovation in the instrument used to harvest fat graft to use in patients with breast cancer and sent to Instituto Nacional de Propriedade Industrial as utility model patent. Method: Search of anteriority and trademarks were performed at internationals and nationals databases, and a prototype was built as a fat harvest instrument with 35 cm length, 4 cm of diameter, 10 holes at the distal extremity, with 2mm diameter each. It was performed an experiment with harvest of a viscous substance, comparing the prototype with 2 regular industry cannulas type: 3 holes Mercedes’s type and 5 roles Pitangy’s type. Results: Four patents of high relevance, and 1 patent of media relevance were founded, and were different from the prototype propose in the distal extremity design or harvest function. When the efficiency of the prototype was compared with the other cannulas, the cannula with 5 holes Pitangy’s type was the most efficient, and it was not founded statistic difference between the prototype and the cannula with 3 roles Mercedes’s type. Conclusion: A prototype with 10 holes at the distal extremity with 2 mm each was built, developed for an efficient harvest of fat blocks until 2mm of diameter, to use in breast reconstruction.

Observa-se que a qualidade dos conteúdos sobre esse procedimento disponibilizados ao público leigo é baixa. É notória a carência de um instrumento de comunicação com teor científico entre profissionais da área da saúde e o público leigo, versando sobre lipoaspiração. Objetivo: Elaborar e validar livro sobre lipoaspiração destinado ao público leigo com base em evidências científicas. Método: A pesquisa seguiu a metodologia Design Thinking. Em Descobrir, foram realizadas busca de anterioridade, levantamento bibliográfico em duas etapas e seleção de artigos. Em Definir, os artigos selecionados foram organizados e os estudos primários tiveram seus níveis de recomendação avaliados. Em Desenvolver, foi produzido o protótipo do livro e realizado o processo de validação por especialistas e pelo público leigo. Em Entregar, foram elaborados comunicação visual, registro e divulgação do produto. Resultados: Foram encontrados 18 materiais semelhantes sobre o tema proposto, sem aplicação de metodologia científica em suas produções. Na primeira e segunda etapas do levantamento bibliográfico, 23 e 26 artigos foram, respectivamente, selecionados para avaliação, havendo apenas 8 com força de recomendação elevada. Alcançou-se a validação do produto tanto por especialistas, quanto pelo público leigo. O livro foi registrado e divulgado de forma online. Conclusão: Foi elaborado e validado um livro sobre lipoaspiração destinado ao público leigo com base em evidências científicas.

Consecrated works in the scientific literature are not unanimous in answering the causes of death related to liposuction. Other researches enquire why the reported causes lead to death in this surgery and not often in other elective surgeries. Several authors seek to clarify what leads, generally young and healthy people, to death in an elective surgery of cosmetic interest. The recognition of causes of death is associated with prevention and in the future may help in establishing prophylactic measures and guidelines. Objective: Identify the patients who died in surgeries related to liposuction in Brazil, determine the causes of death in these surgeries, according to the death certificate and the information published by the press. Identify the time between surgery and death, identify the institutional location in which the surgeries were performed, verify the specialty of the doctor who signed the death certificate and if the same was responsible for the surgery, analysis of the death certificates by an independent researcher who is specialized in necropsy exams. Methods: This is a documentary, quantitative-descriptive research. The data sources were; the media and the death certificates of the people reported. The media provided the name and city of the death. With these data it was possible to collect the death certificates in the civil registry offices. Other sources consulted were the Federal Council of Medicine and Societies of medical classes. Results: Among the causes cited by the death certificates, the indeterminate ones represented 44.18% or 38 cases, pulmonary thromboembolism appears in second place with 17.44% or 15 cases and perforation with 13.95%, 12cases. The most affected age group is between 21 and 50 years old, with 88.23% of cases. The deaths reached 98.04% of females and occurred within the first week after surgery in 82.82%. The deaths occurred in all regions of the Brazilian territory, being pointed out in 20 states and in the Federal District. In 61.76%, liposuction was performed in isolation. Conclusion: The causes of the deaths related to liposuction surgeries could not be identified in approximately half of the cases. The filling of cause of death in the incomplete and sometimes uncharacterized form of death made it difficult to identify them, and this allows us to conclude that the surveys by this source do not satisfactorily provide information on the causes of death in these surgeries. An effective method for the real knowledge of the causes of deaths related to liposuction needs to be created in Brazil. Knowledge of these causes is essential for scheduling prophylactic measures to avoid further fatalities.

Some of its complications can determine life threatening situations, as one can experience with the abdominal viscera perforation. Objective: Elaborate a review about perforation of the abdominal wall and abdominal viscera caused by suction lipectomy. Methods: A search in the scientific information services Pubmed, LILACS, SCIELO, COCHRANE and Google scholar was performed, with the choosen keys words and oficial terms related to the topic Results: The study selected 41 articles, among revisions, surveys, series and case reports. From this selection, 29 articles reported 61 cases about abdominal perforation during liposuction, 12 articles contributed with general information and recommendation about the clinical experience reported in this field. The mortality rate due to abdominal wall perforation during liposuction was 19,6%, and increased to 26,8%, when intestinal perforation was associated. The most common injury was the small bowel perforation. The anterior median abdominal wall was the most frequent muscularaponeurotic layer injured by the suction cannula. Perforation of hernia was reported only in one case in the entire series. Conclusion: A narrative review about the abdominal wall and abdominal viscera perforations during liposuction was performed. Little prevalence of the traditional risk factors was noticed. Important aspects of the clinical practice of liposuction were reinforced in the prevention and in the treatment of abdominal perforation after liposuction.
