- ItemAcesso aberto (Open Access)Hematoma perirrenal espontâneo em paciente lúpica submetida a tratamento hemodialítico e portadora cistos renais adquiridos(Sociedade Brasileira de Nefrologia, 2013-06-01) Loureiro, Juliana Lins; Mendonça, Katienne Goes; Pacheco, Georgia De Araújo; Soutinho, Maria Fernanda Lucena; Presídio, Gustavo Álvares; Ferreira, André Felipe Dos Santos; Silva, Agenor Antônio Barros Da; Ressurreição, Fernando Melro Silva Da; Gouveia, Ebeveraldo Amorim [UNIFESP]; Oliveira, Carlos Alexandre Ferreira De; Medeiros, Marcio Fernando Costa; Bernardo, Rogério César Correia; Universidade Federal de São Paulo (UNIFESP)The spontaneous perirenal hemorrhage usually presents as sudden pain in the side ipsilateral, no history of trauma. Acquired cystic kidney disease is a common finding in chronic hemodialysis patients. However, spontaneous bleeding from the rupture of the cyst is a rare clinical entity. We describe the case of a female patient, 45 years old, with controlled hypertension for 8 years, chronic renal failure for 15 years and lupus nephritis 2 years ago, undergoing hemodialysis three times a week since 2006, and who presented concurrently acquired cystic kidney disease. She was admitted to the emergency department complaining of sudden onset of pain in the thoraco-abdominal left. Perirenal hematoma was diagnosed by ultrasound and computed tomography of the abdomen. The patient underwent embolization of left renal artery, with good evolution.

Hemorrhage is the most common complication of this procedure. In this study, we present the incidence of postoperative hemorrhage following adenotonsillectomy carried out at Hospital Geral de Pirajussara (HGP) and Hospital Estadual de Diadema (HED), whose purpose was to identify cases of bleeding. STUDY DESIGN: Historic transversal cohort. MATERIAL AND METHOD: We analyzed 397 patients who underwent adenoidectomy, tonsillectomy and adenotonsillectomy. Adenotonsillectomy represented 91.7% of all surgeries. Patients were aged 2 to 39 years. Preoperative tests were conducted in all patients, with weekly follow-up up to the first month after surgery. RESULTS: 397 surgeries were performed in the hospitals (HGP and HED), 364 adenotonsillectomies (91.7%), 16 tonsillectomies (4.03%) and 17 adenoidectomies (4.28%). There were 5 cases of postoperative bleeding, 3 of which occurred in the early postoperative period and 2 in the middle postoperative period. There were no reports of bleeding in late postoperative period. The incidence of hemorrhage was 1.37% (5 out of 364 cases). CONCLUSION: Postoperative hemorrhage possibly results from poor surgical technique and blood clotting disorder in some patients.
- ItemAcesso aberto (Open Access)Hemorrhagic cerebral metastasis as a first manifestation of a hepatocellular carcinoma: case report(Academia Brasileira de Neurologia - ABNEURO, 1998-09-01) Peres, Mario Fernando Prieto [UNIFESP]; Forones, Nora Manoukian [UNIFESP]; Malheiros, Suzana Maria Fleury [UNIFESP]; Ferraz, Henrique Ballalai [UNIFESP]; Stávale, João Norberto [UNIFESP]; Gabbai, Alberto Alain [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)We report herein a rare instance in which a patient presented with a hemorrhagic cerebral metastasis as the initial manifestation of a hepatocellular carcinoma (HCC). A few cases of cerebral metastasis from HCC have been reported in the literature, mainly from eastern countries. This is the first report from South America of a cerebral metastasis from hepatocellular carcinoma.
Among its possible complications, there are the thromboembolic events, although uncommon, they are potentially serious. Objective: To evaluate the risk of bleeding and bruising in the postoperative in patients undergoing classic abdominoplasty using low molecular weight heparin (LMWH). Methods: The sample consisted of 28 patients who undergoing classic abdominoplasty divided into two groups: LMWH group (15 patients receiving low molecular weight heparin with a dose of 40 mg per daily for 07 days postoperative) and a control group (13 patients who received mechanic prophylaxis for thromboembolic events). Patients were monitored on daily output drain basis for 07 days, subject to preoperative hematocrit and hemoglobin control, on the 1st, 2nd and 7th postoperative days. Additionally, it was performed an abdominal ultrasound on the 7th PO. Results: In the 1o PO or LMWH group showed a lower drainage output than the controlled group, no statistical difference in the remaining days evaluated. The decrease in hemoglobin and hematocrit was greater in the 1o PO day; there was no statistically significant difference in the total decrease (preoperative compared with 7o PO). There was no hematoma formation in any of the cases studied. Conclusion: There was no risk increase of bleeding and hematoma formation with the use of LMWH in prophylactic doses in the studied cases.
