Methods: We conducted a cohort study by recruiting 59 patients diagnosed with a fracture of the middle third of the clavicle. Information as cause of the trauma, the patient?s activity level, and the epidemiological features was collected. All patients were treated with a figure-?of-?eight bandage until clinical and radiological findings indicated healing of the fracture. The functional outcome was measured using the Disability of Arm, Hand and Shoulder (DASH) score, and the pain, by the visual analogue scale (VAS). The results of our findings are expressed as the comparison of the functional outcome with the degree of shortening. Results: The functional outcome as assessed by the DASH questionnaire averaged 28.84 at six weeks and 8.18 one year before injury. The VAS of pain at six weeks and one year averaged 2.57 and 0.84, respectively. Clavicle shortening due to injury and treatment averaged 0.92 cm, ranging from 0 to 3 cm (SD = 0.64). There was no correlation between the degree of shortening and DASH score of limb function after six weeks and one year (p = 0.073 and 0.706, respectively). When only cases of shortening greater than 2 cm were assessed for correlation, the result did not change. Conclusion: We conclude that clavicle shortening that occurs after conservative treatment with a figure-?of-?eight bandage does not affect limb function in one year follow up.
- ItemSomente MetadadadosCorrelação entre o limiar de dor à pressão e a intensidade de dor em individuos com disfunção temporomandibular submetidos a tratamento conservador(Universidade Federal de São Paulo (UNIFESP), 2014-12-18) Lalue, Monique [UNIFESP]; Alonso, Luis Garcia Alonso [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)Pain is a sensation or unpleasant emotional experience associated with actual or potential tissue damage, which features the most prevalent clinical symptoms in individuals with temporomandibular disorders (TMD). However, its quantification is still precarious regarding existing methods, because although they are well established are not very reliable to assess the clinical course of patients. Objective: To measure at different times, with a digital pressure algometer accuracy, the threshold of pain on pressure in masseter and temporalis muscles of TMD patients, who underwent conservative treatment. Methodology: To date were evaluated 27 individuals who had possessed DTM and tenderness in the masseter and temporalis muscles. After diagnostic measurements were performed pain threshold to pressure of these muscles on both sides of the head and the lateral pole of the temporomandibular joint, by placing the pressure algometer in the region being evaluated. For each individual, was established as a conservative treatment that included therapeutic and thermal therapy exercises. These individuals were reevaluated after 15 days and new measurements were obtained. Also will be evaluated after 45 and 75 days of starting treatment. Results: A total of 21 (77.8%) patients adhere to treatment. The Wilcoxon test to evaluate the improvement of the pain threshold to pressure in the first 15 days of treatment in each measured region, both on the right as on the left side of these patients showed statistically significant differences in the measurements. Right masseter Z = 2.24 (p = 0.0250), right temporal Z = 2.13 (p = 0.0325), for the right side Z = 2.55 (p = 0.0106), left masseter Z = 2.74 (p = 0.0060), left temporal Z = 2.27 (p = 0.0228) and left lateral pole Z = 3.43 (p = 0.0006). Conclusion: Based on these results we can infer that the imposition of a conservative treatment in patients with TMD shows positive results in increasing the pressure pain threshold.
- ItemAcesso aberto (Open Access)Tradução, adaptação cultural e aplicação de um questionário de dor para pacientes com doença renal policística(Sociedade Brasileira de Nefrologia, 2010-12-01) Eloi, Samara Rodrigues Moreira [UNIFESP]; Nishiura, José Luiz [UNIFESP]; Heilberg, Ita Pfeferman [UNIFESP]; Universidade Federal de São Paulo (UNIFESP)INTRODUCTION: Pain is a common symptom in patients with autosomal dominant polycystic kidney disease (ADPKD), affecting around 60% of cases. OBJECTIVE: Translate a pain questionnaire developed and validated for ADPKD in USA into Portuguese and to perform its cultural adaptation and apply it. METHOD: The cultural adaptation performed by a panel of experts resulted in small changes consisting of words substitution by synonyms or deletion of terms not commonly used in our culture in 12 out of the 46 questions posed, to solve patients difficulties in understanding the questionnaire. RESULTS: There has been equivalence between the adapted form of the instrument with the back-translation. The final form of the questionnaire applied in 97 patients with ADPKD (64F/33M, 35 ± 12 years) showed that 65 (67%) had isolated or associated pain in multiple locations , more often at lumbar region (77%), followed by abdominal (66%), headache (15%) and chest (4%). The questionnaire revealed that after family history, pain was the second factor contributing to the diagnosis of ADPKD in this population (55% and 22% of cases, respectively). DISCUSSION: Clinical and laboratory data from medical records showed that patients referring pain had renal volume and size of the largest cyst significantly higher than those without pain. CONCLUSION: We conclude that the use of a specific pain questionnaire for ADPKD population provided a better characterization of this symptom, as well as its relationship with the associated complications that commonly occur in this setting.
