They present high morbidity and mortality, with high rebleeding rate and difficulty of surgical approach. We present a 19 years old man who was victim of physical aggression in the occipto-cervical region, presenting subarachnoid hemorrhage and a dissecting aneurysm of the right vertebral artery, which had a fenestration, being submitted to endovascular treatment. We accomplished a literature review about this subject, proposing endovascular treatment as a therapeutic option for these cases.

Since 1948 few cases were described in the literature. We report on a 33 year-old female patient with subarachnoid hemorrhage due to sacular aneurysm of the left AICA. She was submitted to clipage of the aneurysm without complications.

Occasionally, aneurysms are not amenable to the standard femoral approach. We describe four cases of basilar artery aneurysm that were treated by the direct vertebral artery access of V1 at the cervical region. The direct vertebral artery access technique can provide an alternative route in selected cases for the treatment of basilar artery aneurysms.

Treatment strategies have included anglography and craniotomy. However, with advancement of non-operative interventions, much controversy surrounds the treatment of these patients. Endovascular embolization has been used in many patients with cerebral aneurysms, but it had been reported in the literature in only two sickle cell patients. the authors present a case of endovascular embolization of multiple intracranial aneurysms in a patient with sickle cell anemia with good outcome. In order of this, samples of 254 patients with diagnosis of intracranial bleeding were analyzed, with special attention to the rate of CSF total proteins. Statistical tests for evaluation of the results have been accomplished, revealing a close relationship between the total proteins rates increase and death in patients with subarachnoid hemorrhage, independent of sex and age. The limit score of total proteins level for survive was 3000 mg/100 ml (nephelometric method).
