O funcionamento dos bancos e postos de coleta de leite humano da região metropolitana de São Paulo: estudo transversal

Resumo
O presente estudo teve como objetivo analisar o funcionamento dos Bancos de Leite
e Postos de Coleta de Leite Humano da Região Metropolitana de São Paulo. Trata se de um estudo transversal com 22 Bancos e 8 Postos de coleta com cadastro ativo
na Rede Brasileira de Bancos de Leite Humano em 2017. As variáveis estudadas
foram as características dos Bancos e Postos de coleta, segundo recursos humanos
e funcionamento; dados de estatística vital censitária relativas a 2017; e dados
relativos à coleta domiciliar do leite humano. Utilizou-se um software denominado
Research Eletronic Data Capture® para a coleta dos dados. Os resultados
mostraram que a Rede Regional de Atenção à Saúde 6 sobressaiu-se no número de
unidades e de atividades realizadas e, na Rede Regional de Atenção à Saúde 3, não
foi identificada nenhuma unidade nesta pesquisa. Observaram-se discrepâncias
entre os índices de prematuridade, de leitos de terapia intensiva neonatal, e
unidades de Bancos e Postos de coleta. As instituições privadas tiveram o maior
número de receptores, doadoras e maior distribuição de leite humano em suas
unidades. As instituições públicas são as únicas que fazem a coleta de leite humano
de forma domiciliar e captam maior volume do que distribuem em sua maioria, além
de ter maior número de profissionais exclusivos, o que não repercute nos resultados
relacionados ao quantitativo de assistência, à coleta e distribuição de leite humano.
As dificuldades mais citadas para a realização da coleta domiciliar foram a escassez
de recursos humanos, falta de transporte, distância e trânsito. As sugestões mais
citadas para a melhora estão relacionadas aos aspectos estruturais e de recursos
humanos. Conclui-se que apesar de a Região Metropolitana de São Paulo ser o
local com a maior concentração de Bancos e Postos de coleta de leite humano do
Brasil, constatou-se que há lacunas e inconsistências na distribuição geográfica, na
gestão de dados de trabalho, no aproveitamento e cobertura do leite humano e da
coleta domiciliar entre as Redes Regionais de Atenção à Saúde e as unidades de
gestão pública e privada. Há necessidade de aperfeiçoamento no planejamento dos
serviços, gerenciamento dos dados de trabalho, inserção para áreas descobertas e
investimentos em recursos humanos e infraestrutura pelos gestores públicos para a
melhora dos dados de trabalho e assistência nos Bancos e Postos de coleta de leite
humano.
This study aimed to analyze the functioning of Milk Banks and Human Milk Collection Points in the Metropolitan Region of São Paulo. This is a cross-sectional study with 22 banks and 8 collection points with active registration in the Brazilian Network of Human Milk Banks in 2017. The variables studied were the characteristics of banks and collection points, according to human resources and functioning; vital census statistics data for 2017; and data related to the collection of human milk at home. A software called Research Eletronic Data Capture® was used for data collection. The results showed that Regional Health Care Network 6 stood out in the number of units and carried out, and in Regional Health Care Network 3 in the research no one unit was identified. Discrepancies were observed between the rates of prematurity, neonatal intensive care beds, and banks and collection points units. Private institutions had the largest number of recipients, donors and greater distribution of human milk in their units. Public institutions are the only ones that collect human milk at home and capture a greater volume than they mostly distribute, in addition to having a greater number of exclusive professionals, which does not affect the results related to the quantity of assistance, to the collection and distribution of human milk. The most cited difficulties in carrying out household collection were the lack of human resources, lack of transport, distance and traffic. The most cited suggestions for improvements are related to human resources and resources. It is concluded that although the Metropolitan Region of São Paulo being the place with the highest concentration of human milk banks and collection points in Brazil, it was found that there are gaps and inconsistencies in their geographic distribution, in the management of work data, non-use and coverage of milk collection between the Regional Health Care Networks and the public and private management units. There is a need for improvement in the planning of services, management of work data, insertion into uncovered areas and investments in human resources and infrastructure by public managers to improve work data and assistance in banks and human milk collection points.
